We are seeking an experienced Travel Physical Therapist (PT) for a home health assignment. The PT will provide in-home therapy services, helping patients improve mobility, function, and independence while managing a flexible caseload.

Assignment Details

- Schedule: Monday–Friday, 8:00 AM – 5:00 PM

- Shift: 5x8 Days

- Length: 13 weeks

- Region: East (Baytown and surrounding areas)

- On-Call: None

- Guaranteed Hours: 40

Responsibilities

- Provide physical therapy services in a home health setting

- Evaluate patients and develop individualized treatment plans

- Deliver therapeutic exercises, mobility training, and functional rehabilitation

- Monitor patient progress and adjust care plans as needed

- Communicate with patients, families, and care teams regarding care plans

- Report missed visits and patient contact issues per protocol

- Maintain accurate and timely documentation using WellSky

Productivity & Travel

- Point system: 1 visit = 1 point

- Weekly expectation: 32–34 points

- Paid drive time and GSA mileage reimbursement

- Drive time is included in scheduling and may qualify for overtime if exceeding 40 hours

Requirements

- Required: Minimum 2 years recent licensed PT experience (no new grads)

- Required: Home health experience

- Required: Active PT license in state of assignment or ability to obtain

- Required: BLS & First Aid certification

- Charting System: WellSky

Additional Information

- Candidates must be prepared to secure housing prior to submission
