This job provides individualized, direct patient care of assigned patients by performing a variety of sonographic procedures and/or tests, not limited to electrocardiograms and associated stress tests, echocardiograms to include 2D/M Mode, Doppler and Color Flow imaging, Exercise Stress Echo and Dobutamine Stress Echo. Compiles history and diagnostic information from the examination, and presenting the preliminary sonographic findings. Provides the highest quality personalized patient care and maintains a positive work relationship with the health care team.

Education
Required - High school diploma or equivalent.
Work Experience

Certifications
Required - A ctive RDCS registry through ARDMS Credentialing Agency or RCS registry through CCI Credentialing Agency within 6 months of completing the required 12 month clinical experience period.
Basic Life Support (BLS) from the American Heart Association.

* General knowledge of cardiac anatomy, Doppler physics, and the hemodynamics of blood flow in relation to the various disease states.

As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Primary Responsibilities:
* Clinical Competence
* Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
* Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
* Provides required supervisory visits
* Documentation and Care Delivery
* Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members
* Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
* Documents patient visits per policy and payer requirements, and syncs timely per LHC policy
* Quality
* Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation
* Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians
* Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence
* Teamwork
* Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning
* Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students
* Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes
* Participates in on-call and weekend rotation as needed to meet patient needs
* Adheres to and participates in the agency's utilization management model

Required Qualifications:
* Current and unrestricted RN licensure in state of practice

* 1+ years of clinical experience as a Registered Nurse
* 1+ years of clinical experience as a Registered Nurse may be waived for a Registered Nurse with recent clinical experience as an LPN

This job, as a part of a multidisciplinary team, assumes the responsibility for the operation of the electrophysiology suite. Assists physicians in diagnosing and treating cardiac arrhythmias in the electrophysiology (EP) lab, specializing in the care of patients undergoing EP studies, device implantation and ablations. Operates diagnostic equipment, monitors patient vitals, performs routine calculations, patient circulation, and assists the physician, as needed, during the procedure. Prepares the procedure rooms, equipment, and patient for the procedure. Acts as liaison to the hospital Radiation Safety Officer (RSO).To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Assists the team in patient setup and prep before the procedures and take down/clean up post-procedure using safe patient transfer and positioning techniques
Maintains proper sterile technique to assist the physician with implantation of cardiac devices.
Operates complex electrophysiology specialty equipment such as EP Hemodynamic, PRUCKA, and stimulator.
Operates radiographic fluoroscopy equipment to obtain diagnostic images
Analyzes and files electrocardiographic data in picture archiving and communicating systems (PACS).
Performs quality control standards by using the optimum diagnostic imaging and performing regular maintenance of fluoroscopy equipment.
Monitors delivery of radiation dose to patient to achieve safe delivery with minimal exposure to staff.
Participates in activities for professional development and maintains required clinical knowledge, technical skills, training, and credentials.
