Responsibilities:About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
- Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
- Administer and order point of care and lab-based testing as needed
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
- Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

Requirements:
- Drivers must have a current medical card and Class A CDL with doubles and triples endorsement.
- No CDL restrictions.

- Reports malfunctioning equipment and disturbing incidents that occur in patient rooms and on the unit.
- Charges supplies appropriately, prevents waste of supplies.
- Cleans specified equipment and areas as assigned. Removes trash and used equipment.
- Maintains patient records and charts to ensure an accurate record of the patient's medical treatment including temperature, blood pressure, weight, respiration and pulse.
- Provides personal care assistance to patients, including bathing, dressing, eating, bedpans and transportation.
- Maintains confidentiality of patient and unit care data.
- Participates in educational programs and in-service meetings.
- Orders supplies and equipment as delegated by Nurse Manager.
Requirements:
Education:
- High school diploma or equivalent.
Experience:
- One year Nursing Assistant experience preferred and/or completion of Nursing Assistant course within one year. Two years hospital experience preferred.
- Completion of competency based skills checklist.
Certifications, Registrations, or Licenses:
- BLS certification required.
