Key Responsibilities;


  • Serve as the primary nurse for members requiring lifestyle guidance, acute symptom support, chronic care management, complex condition management, or end-of-life navigation.
  • Conduct telephonic outreach, coaching, and case management to help members develop self-management skills and improve adherence to evidence-based care.
  • Perform initial and ongoing assessments to identify member needs, risks, barriers, and opportunities for intervention.
  • Develop individualized care plans with measurable goals and provide follow-up to evaluate progress and adjust interventions.
  • Provide clinical coaching to members calling with acute symptoms, chronic condition questions, or general health inquiries.
  • Coordinate with physicians and healthcare teams to facilitate communication and ensure appropriate, timely care.
  • Identify member barriers to treatment adherence and support behavior change and lifestyle improvements.
  • Connect members with community, online, and telephonic resources; assist with accessing covered benefits and services.
  • Monitor service quality based on member feedback and report safety or quality issues as needed.
  • Maintain accurate and timely documentation consistent with regulatory, accreditation, and organizational standards.
  • Participate in ongoing training and educational programs to maintain licensure and enhance Health Coach competency.
  • Provide exceptional customer service with a supportive, member-centered approach.
  • Perform other duties as assigned.


Key Requirements and Technology Experience:


  • Current, active, unrestricted Pennsylvania RN license (multistate/compact license acceptable if it includes PA).
  • Minimum 3 years of combined experience in clinical nursing, case management, disease management, or chronic/complex condition management.

The Division of Hematology and Cellular Therapy of the Allegheny Health Network Cancer Institute, primarily located at West Penn Hospital in Pittsburgh, seeks a highly skilled and compassionate physician leader with expertise in Hematology and Cellular Therapy (HSCT and CAR-T). This position offers a Division Chief leadership role in a growing and dynamic Hematology and Cellular Therapy program dedicated to clinical and translational research as well as exceptional patient care.


The Division Chief works with the AHN Cancer Institute Chair and other leaders to develop and implement strategies and processes for patients under the care of the Division of Hematology and Cellular Therapy. The Division Chief shall also maintain all current and future standards of care, both inpatient and outpatient, as well as accreditations needed to assure service for the region. The Division Chief will maintain the same standards and accreditations for subdivisions of the Cell Processing and Molecular Hematology Laboratory. The Division Chief will assume responsibilities for research and development within the division and connect these activities to the larger goals of the AHN Cancer Institute. The successful candidate will be considered for the role of Division Chief of the Hematology and Cellular Therapy Program.

RESPONSIBILITIES

  • Develop and execute a business plan that will establish activity levels and target account acquisitions for the upcoming year to allow you to achieve your sales goals.
  • Focus on new account acquisition, targeting local builders, contractors, remodelers, and replacement companies to drive results and meet established goals.
  • Regularly visit permit offices or review permit reports to uncover active new projects and accounts in your area.
  • Participate in select functions, such as trade shows or chapter meetings, to build your network and relationships with prospective clients.
  • Continually update and leverage knowledge of Pella and competitor products.
  • Complete take-offs when applicable; Develop window and door solutions to satisfy prospects and customers’ needs.

Key Responsibilities:

- Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
- Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
- Develop and implement evidence-based treatment plans tailored to the needs of each patient.
- Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
- Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
- Monitor patient progress and adjust treatment plans as needed.

Requirements:

- Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
- Effective oral and written communication in English
- Basic proficiency with technology, including electronic health records (EHR).
- Strong organizational and documentation skills, with attention to regulatory compliance.
- Ability to provide in-person services at assigned facilities, up to 45 minutes.
- Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
- Full Vaccination and Booster Status may be required in some facilities.

Preferred Qualifications:

- Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
- History of treating Anxiety, Depression and Adjustment Disorders.

- Psych or Addictions experience is a plus.
