Mount Sinai Health System is seeking a Senior Medical Director of Quality and Patient Engagement.
The Senior Medical Director (SMD) for Population Health Quality and Patient Engagement drives the implementationand ongoing improvement initiatives that enable Mount Sinai to deliver better value to its patients, its customers (i.e.,plan sponsors and payers), its providers, and its partners. At Mount Sinai, value is defined broadly and encompassesimproved health outcomes, more efficient operations, better patient experience, more joyful care team environment,and improved financial performance in value-based contracts.
MSHP seeks a Medical Director (MD), Physician Engagement and CDQI, who will (1) be responsible for driving key population health goals including access, quality, utilization and accurate risk adjustment with a group of practices and (2) close partnership with the CDQI Director to drive accurate risk adjustment throughout the system. This Medical Director must be effective at influencing physicians, medical director and other key practice staff to drive value based care success. He/she will support the establishing and hardwiring processes to drive excellence in efficiency of care, quality and accurate risk adjustment for all populations served, especially our patients covered under a Medicare arrangement (MSSP or MA). The MD will report to Vice President, Clinical Integration and Population Health with shared accountability to Director, CDQI.
SMD
The primary focus of the SMD will be:
- Select Employed Ambulatory Metrics
- Select Voluntary Ambulatory Metrics
- Epic Optimization to Support Above
- Primary care patient engagement rates
- A mission critical metric for care excellence and primary care success)
- Focused on all payer classes, with deeper focus on patients who have Medicare and Commercial insuran
This SMD will report to the CMO, Population Health and Clinically Integrated Network. The SMD will partner with the
MSHP analytics team, Digital and Technology Partners (DTP, i.e. IT) Epic EMR team to drive targeted quality and patientengagement performance most relevant to Mount Sinai's value based care contracts and select system quality goals(defined with the CMO of Population Health and CIN, and other key system stakeholders.
The ideal candidate will have significant experience (5+ years) in leading quality initiatives in the ambulatory setting, and
a record of accomplishment in meeting or exceeding quality metrics. Leadership experience in ACO or value-based carequality metrics is required. Key personal attributes include a positive and enthusiastic attitude, efficient and focus onkey results, and a balanced mix of confidence and humility.
Key Expectations also include:
- Co-develop strategy with team and CMO, Population Health and CIN,
- Identify and track performance on key indicators,
- Analyze performance against national benchmarks,
- Plan and co-develop content for physician education/engagement initiatives
Qualifications
- Unrestricted license to practice medicine in New York State
- Board-certified and in good standing with a minimum of 3 years clinical experience and at least 3 years of
- demonstrated administrative and leadership responsibilities
- 3+ years of experience and shareable result within ambulatory quality/value based care quality
- Familiarity with Epic EMR and Some Community EMRs, strongly preferred
- Knowledge of MACRA/MIPS quality, strongly preferred
Compensation range from 230K-270K (not including bonuses / incentive compensation or benefits)

About the facility: The Williamson, NY operation is a roughly 1 million sq ft production facility with 217 acres of land. The site is the Home of the Brand Motts that has been delighting our consumers for more than 140 yrs. We produce a wide variety of products with diverse packaging capabilities going from Apple sauce, to juices and concentrates . Alongside production, the site includes a raw apple operation that is key for the success of the site. This facility is a Unionized location. Production Operations Manager
The Operations Manager will be responsible for all manufacturing personnel, process, materials and equipment from start to end. This individual is accountable for delivering the established site goals, as well as collaborating with site leadership to establish and lead processes that achieve high standards of safety, quality, productivity, cost control, and high employee engagement. Our Williamson facility runs 24/7 creating our famous MOTTS products as well as some of our other delicious brands and flavors of beverages. This role is a Non-union role; however, our production team is Unionized. This individual will be heavily involved in Labor Relations.
Shifts & Schedule: This full-time position will be supporting our Night Shift. Additional flexibility to work overtime.
Position Responsibilities

- Leverages Lean, 5S, and TPM principles to drive process improvement across all aspects of manufacturing including safety, quality and productivity. Identify, recommend and execute continuous improvement initiatives in the production operation including process, machinery, method and material changes, in support of plant goals. Develop and streamline procedures for coordination of supply chain management with other functional areas.
- Proactively engages maintenance and technical functions to work in unison with operations team members to maintain equipment in OEM conditions safely, efficiently, and cost effectively with minimal disruption to customer service.
- Drives employee engagement using HPT principles; Sets goals, assesses performance, ensures open communication, provides coaching, and establishes a team atmosphere.
- Develops procedures, protocols, and standardized work; Ensures/Provides training to develop/grow direct reports.
- Manage all employee related issues within the department. Effectively manage departmental staffing requirements in accordance to the operating budget.
- Guide and support technicians in trouble-shooting production equipment when necessary.

- 2 years of experience in an Operations Management role within a manufacturing environment
- 5 years of experience in a dynamic, fast-paced manufacturing environment required

- 2 years of experience with lean, kaizen/six sigma events and driving lean transformation with proven results and in a manufacturing environment (5S, Autonomous Maintenance, Planned Maintenance, visual management, and leading Kaizen events)
- Experience in SAP preferred

Reporting Structure: Direct Report to Catholic Health (CH) Chief Clinical Officer for clinical direction and the CH Chief Operating Officer for operational, financial, business, and programmatic planning.
Clinical and Administrative Split: (.7T Clinical and .4 Administrative)
- The Chair is responsible for the leadership and coordination of the Service Line across the six hospitals and all ambulatory service sites.
- The Chair will interface with other System leaders to integrate emergency care and oncall coverage and facilitate resources as appropriate for full cross continuum care.
- The Chair will work in partnership with the Vice President, Orthopedic, and Musculoskeletal Service Line. In this dyad relationship, both the Chair and the Vice President will lead planning and implementation of strategic initiatives within the service line, enhance practice protocols, advise on value based care strategies, as well as equipment acquisitions.
- Develop and oversee performance measures that build on and improve the CH system.
- The Chair will also be responsible for interfacing with referring physicians to promote the Musculoskeletal Service Line.
- The Chair and the Vice President, working in collaboration with regional and local executive and hospital leadership will set strategic vision to ensure execution for the Service Line.
- The Chair will have accountability for the Service Line’s clinical care delivery, recruitment, research growth, educational programs and community service. He/She will also be responsible for the transformation of the Service Line into the Musculoskeletal Institute at CH.

- Establish staff and physician recruitment strategy based on budget and volume needs, including succession planning.
- Develop evidence-based protocols and build COEs System wide. Work with Hospital Chairs/Chiefs/Presidents to implement protocols and develop/maintain COEs.

- Prioritize and align campus-specific expansion opportunities identified by local clinical leaders to fit into service line and CH strategic priorities

Responsibilities
- Provide construction management inspection for multi trade contracts to ensure the project is constructed in conformance with the contract requirements
- Monitor work and materials to ensure quality control standards are met at various stages of the project
- Coordinate Third Party Code Inspection and Documentation
- Communicate with the project team and support each subcontractor to execute his job duties effectively and efficiently
- Perform inspections or schedule inspectors to meet the varying safety and compliance regulations of each job location
- Maintain a written daily log of visitors, job actions performed, materials expended, and problems resolved on the job site
- Identify and resolve problems and conflicts, organizing meetings and cooperating with team members in various roles as necessary
- Oversee safety and security at project sites
- Review contractor as-builts
Qualifications
- Minimum (6) years of experience in construction involving multiple phases of large-scale building/rehabilitation projects
- Minimum (4) years of experience as a Superintendent or Inspector on large scale, multi-contract building project
- Ability to develop/communicate a project work plan, monitor manpower and performance against that work plan, and be able to perform a detailed comparison of the design drawings against each other to ensure coordination between each of the various project trades
- Strong communications skills and a collaborative approach working with both client and construction contractor/subcontractors
- Strong interpersonal and leadership skills
- Experience with Submittal Exchange and Bluebeam preferred
- Experience in Institutional Construction preferred
