• Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient's outcomes by building a trusting relationship and helping them change behaviors.
• Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients.
• Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals.
We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP's become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education.
- Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality and support systems. Serves as patient advocate.
- Patient management includes the following:
- Writes admission, transfers and discharges orders.
- Orders and interprets appropriate laboratory and diagnostic studies.
- Orders of appropriate medication and treatments.
- Refers patients for consultation when indicated i.e. dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc.
- Documentation through in-depth progress notes and summaries.
- May perform invasive procedures independently upon the completion of documented competency.
- Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.
- Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary.
- Uses advanced communication skills to problem solve complex situations and to improve processes and service to patients.
Other Responsibilities may include:
- Collaborates with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate.
- Participates in outside activities that enhance personal and professional growth and development.
- Initiates arrangements and writes orders for SNF discharges and completes appropriate paperwork.
- Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate.
- Introduces self to patient/family and explain nurse practitioner role.
- Facilitates patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify post-SNF needs.
- Enhances a collaborative relationship to maximize the patient's/family's ability to make informed decisions re: goals of care, palliative care and hospice.
- Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.
- Prescribes medication to patients based on State of practice.
- Other duties as assigned and modified at manager's discretion.

- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates

PRINCIPAL RESPONSIBILITIES AND DUTIES
CLINIC OPERATIONS:

The Senior Associate, Broadcast Investment is responsible for all aspects of broadcast media management (audio and video) for assigned clients, including negotiation, placement, management and reconciliation for assigned clients. Part time RN Case Manager, Emergency Dept
20 hours/week
CARE COORDINATION: The RN Case Manager works with the multidisciplinary team including physicians, staff, and payers to ensure patient's progress along the continuum of care in an efficient and cost-effective manner that ensures quality outcomes. The RN Case Manager acts as a patient advocate and communication link with other departments and community resources. Supports nursing staff in development of individualized, evidence-based, outcomes oriented, and safe practice
Responsible for patient assessment of complex clinical discharge needs and establishment of an appropriate plan of care with multidisciplinary team. Demonstrates critical thinking skills and oversees the care coordination of defined population. Ensures that patient goals are set with patient/family, negotiates healthcare resources and educates patient/family regarding available options of care throughout the continuum. Demonstrates clinical competence to care for a specific patient population. Facilitate patient education for safe and effective discharge.
PROFESSIONAL DEVELOPMENT: Demonstrates leadership behaviors that enhance and support team functions. Mentors and coaches staff. Maintains competencies and participates in continuing education for self and development of colleagues and students. Participates in delivering professional programs/ courses/in-services, internal and/or external, and community events. Seeks opportunities to promote staff education, certification, clinical ladder, and award programs. Maintains national certification, as applicable, and membership in professional organization(s). Creates opportunities to advocate for the role of the professional nurse as a key member of the interdisciplinary team.
POSITION REQUIREMENTS:
Minimum Education: Required: Bachelor's Degree
Preferred Education: Master's Degree
Minimum Experience: Required: 3-5 years of previous job-related hospital experience.
Preferred Experience: 6-10 years of previous job-related hospital and Inpatient Care Management experience
Preferred Certification: ACMA, CMGT-BC, CCM
Recent Case Management experience is preferred.
DETAILS: Recent clinical experience in medicine, surgery, critical care, telemetry, or ED is required.
Compensation:
Salary Range: $35.89 - $62.91 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.

We have an exciting opportunity for a Purchasing Manager. You will oversee all materials management functions, including inventory control, purchasing, receiving, and supplier management. This role ensures that production, maintenance, and operational departments have the materials, tooling, and consumables needed to operate safely, efficiently, and on schedule. The position is responsible for developing local procurement strategies, optimizing inventory levels, controlling costs, and ensuring compliance with internal, customer, and federal-contractor requirements. Responsibilities will include the following:
Purchasing & Procurement
- Lead all purchasing activities for raw materials, consumables, MRO supplies, tooling, spare parts, and contract services in coordination with Corporate Procurement
- Develop sourcing strategies that support cost savings, vendor reliability, and production uptime.
- Negotiate contracts, pricing, terms, and service levels with suppliers.
- Maintain approved supplier lists and assess vendor performance regularly.
- Ensure compliance with federal-contractor purchasing requirements
- Coordinate with engineering, maintenance, operations, and finance to ensure timely purchasing aligned with operational needs and budgetary constraints.
Stores & Inventory Management
- Oversee all warehouse and storeroom operations, including receiving, stocking, issuing, cycle counting, shipping, and documentation.
- Implement inventory control systems to minimize shortages, excess inventory, and stockouts.
- Monitor inventory levels and set appropriate reorder points and safety stock for critical items.
- Ensure accurate records in the ERP system; audit physical vs. system inventory.
- Maintain safe, organized, and compliant warehouse conditions.
Logistics & Supplier Coordination
- Oversee receiving inspections and ensure discrepancies, damage, or non-conformances are resolved promptly.
- Coordinate expediting activities for urgent or critical parts.
Financial & Compliance Oversight
- Create and manage annual budgets for purchasing, inventory, and stores operations.
- Analyze cost drivers and identify opportunities for cost reduction or process improvement.
Leadership & Team Development
- Supervise buyers and stores associates.
- Train and develop team members in procurement processes, ERP use, inventory control, safety, and compliance.
- Establish clear performance metrics and accountability for purchasing and stores teams.
Qualifications
- Bachelor’s degree in Supply Chain, Business, Operations Management, or related field (or equivalent experience).
- 5–7+ years of purchasing, supply chain, or stores/warehouse management experience—preferably in steel manufacturing, heavy industry, machining, or metals.
- Strong negotiation skills and contract management experience.
- Experience with ERP/MRP systems (SAP, Oracle, Epicor, Infor, etc.).
- Knowledge of inventory control techniques
- Strong leadership, communication, and cross-functional collaboration skills.
