City/State Virginia Beach, VA Work Shift First (Days) Overview: Sentara Health Plans is hiring for a Behavioral Health Utilization Management Care Coordinator- Whole Person Care- Hybrid in the Peninsula Region! Position Status: Full-time, permanent (40 hours/week) Standard Working Hours: 8 AM to 5 PM EST, M-F Position Location: This is a hybrid role based in Peninsula Region (Newport News, Hampton, Williamsburg, Yorktown, Gloucester, VA, and the surrounding areas).

The role includes travel of up approximately 2 days per week in the community (home, CSB, or hospital visits as needed) and 3 days working from home.

Overview The position will provide case management for members enrolled in Sentara Health Plans who have both medical and behavioral health complexities.

The population served includes children, adolescents, and adults.

Education Master's degree in Counseling , Social Work, or Marriage and Family Therapy required .

For Registered Nurses, a Bachelor of Science in nursing is REQUIRED Certification/Licensure REQUIRED license must be valid in state of practice.
