Social Worker (PACE) - HOT SPRINGS
Job Description
Department: PACE Hot Springs Shift: Day Working Hours: 8:00am-5:00pm Summary: Responsible for the direct social work case management services to PACE participants.
Provides social service support to include participant screening and assessments, case management, counseling and referral.
Other information: ● Master's degree required ● LMSW or LCSW required ● Two years experience in healthcare preferred ● Experience working with the frail and elderly population preferred ● Excellent customer service skills ● Excellent interpersonal communication skills This job will be authorized 80.00 hours bi-weekly.5c143e31-5e48-4549-b638-05792d185386
