For this position we are seeking a (RN) Registered Nurse who lives in Kentucky and must be licensed for the state of Kentucky Case Manager RN will work with KY Behavioral Health Medicaid population providing telephonic case management support. Job Summary
Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care.
Essential Job Duties
β’ Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management based on clinical judgment, changes in member health or psychosocial wellness and triggers identified in assessments.
β’ Develops and implements care coordination plan in collaboration with member, caregiver, physician and/or other appropriate health care professionals and member support network to address member needs and goals.
β’ Conducts telephonic, face-to-face or home visits as required.

β’ Maintains ongoing member caseload for regular outreach and management.

β’ Facilitates interdisciplinary care team (ICT) meetings and informal ICT collaboration.
β’ Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.
β’ Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
β’ May provide consultation, resources and recommendations to peers as needed.
β’ Care manager RNs may be assigned complex member cases and medication regimens.
β’ Care manager RNs may conduct medication reconciliation as needed.
β’ 25-40% estimated local travel may be required (based upon state/contractual requirements). Required Qualifications
β’ At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience.
β’ Registered Nurse (RN). License must be active and unrestricted in state of practice.
β’ Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law.
β’ Understanding of the electronic medical record (EMR) and Health Insurance Portability and Accountability Act (HIPAA).
β’ Demonstrated knowledge of community resources.

The Region Clinical Intake Specialist is responsible for managing and coordinating the clinical intake process for both Home Health and Hospice services across multiple branches. This role ensures timely, accurate, and compliant intake procedures using the Homecare Homebase (HCHB) system, serving as a key liaison between referral sources, clinical teams, and operational leadership.
Performs all clinical aspects of the intake process, including communication with patients and families and admitting clinicians, clinical data entry accuracy, timeliness and Face to Face encounter accuracy and PCP/MD verification. Ensure patients are admitted timely and in accordance with the intake policy. Provides recommendations for process and policy improvements. Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
Stays thoroughly informed about the details of clinical quality metrics of the organization has with key Home Care Leadership, healthcare networks, or other partners for Home Health and Hospice services. Ensures patient admissions comply with rules, requirements and terms in the contracts.
Establishes and maintains positive working relationships with current and potential referral sources and associated clinical leadership teams.
Provides a smooth coordination of care of patients to home care by overseeing patient education, care planning, and coordination with multiple providers, case managers and other clinical needs within established payer and financial guidelines.
Strengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high-quality service provider, while collecting and reporting clinical referral data and key customer trends.

Essential Job Functions:
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.

The Assistant Clinical Manager β Home Health, will evaluate nursing activities to ensure patient care, staff relations, and efficiency of service within the team.
