The Physical Therapist under the supervision of the Center Manager, provides physical therapy services to program participants. Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.

  • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing
  • Minimum of one-year experience working with a frail or elderly population required.
  • Must clear the background check.
  • Must have a valid driver’s license and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

 


Essentials: 



  • Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
  • Evaluates participant’s needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
  • Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
  • Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

40+ hours a week guaranteed, locations to vary, however the same days off will be pretty consistent. Most of the shifts you work will be over night grave shifts , some of them will be swing and occasionally we will ask you to work days. We do our best to give you a consistent schedule and as we grow other opportunities may become available.


1st Year Hourly Pay: $30.00 + $1.00/hour (uniform maintenance) = $31.00/hr


2nd year Hourly Pay or when you make supervisor, whichever is first: $32.00 + $1.00/hour (uniform maintenance) = $33.00/hr


Vacation Time: 24 hours the 1st year (based on working 40 hours/week) then an additional 8 hours added every year thereafter (2nd year 32 hrs, 3rd year 40 hrs, 4th year 48 hrs, etc.)


Annual Bonus: We pay $100 for the 1st year anniversary and an additional $100 for every year thereafter. (2nd year $200, 3rd year $300, etc)

Job Description: Our Flex Officers are trained to work at every job site within SGT. This includes working mobile vehicle patrol. We depend on our Flex Officers to work a job site whenever there is a call off or a fire watch. Typical job site locations would be; San Francisco, Berkeley, Redwood City, Fremont, San Jose, Mountain View, etc.

The mobile vehicle patrol aspect of this job will have you driving all of the Bay Area in a marked security vehicle (picked up in Newark CA), which is usually a black Ford Explorer with light bar and spotlights. Officers visit many different properties throughout each night. Typical mobile patrol duties are to look for anything unusual, enforce parking rules, scan checkpoints, write detailed reports w/pictures, and respond to calls for service like an alarm or noise complaint.


Summary: Essentially we depend on our Flex Officers for just about everything. We are still a startup company so you must be "ALL IN" on this opportunity, which simply means you are dedicated to do whatever is needed to take care of our clients. Normally we work 8 hour shift, however sometimes this means working 12 hour shifts and working extra days to help cover open shifts. However, our goal is for everyone to be able to make great money and not have to work overtime on a regular basis.

Once a Flex Officer has been shown to be proficient at all SGT job locations, that officer may be promoted to Supervisor and a $2/hour increase in pay. Supervisors are then groomed to become managers.


Job Duties Including But Not Limited To:

  • Drive from property to property in a marked security vehicle.
  • Post check security officers for well being and uniform inspections.
  • Train security officers on post orders and standard operating procedures.
  • Work post sites when officers are late or call off.
  • Answer phones and dispatch the officers in the field for service calls.
  • Respond to service calls.
  • Walk around the property checking areas of interest from Client.
  • Conduct patrol tours of using a checkpoint scanning system with smartphone.
  • Identify potential security risks and report them.
  • Complete incident reports and required reports for all security incidents using a smartphone.
  • Respond to emergencies or incidents and report them.
  • Assist employees and visitors with any questions or concerns they may have.
  • Escalate emergency situations to law enforcement as needed.
  • Meet with Clients.

Become a Weeknight On-Call RN with Georgia Hospice Care

As a registered nurse at Georgia Hospice Care, you’ll evaluate patients and create care plans, all while communicating with everyone involved — the patient, the patient’s family, and the care team. You’ll serve as the driver of our care team to ensure every patient receives quality care. Weeknight On Call RNs are salaried full-time to cover on-call hours of 5 pm - 8 am Monday, Tuesday, Wednesday, Thursday and ending on Friday at 8 am, working alongside our 24/7 365 RN staffed triage department. If and when called upon you will see patients during your working hours.

Position Summary
This position is responsible for coordinating and supporting external recruiting activities and pre-employment processes to ensure an efficient and positive candidate and new hire experience. The External Recruiter manages applicant tracking system activities, external job postings, candidate screenings, interview scheduling, and onboarding logistics while leading internship programs and job fairs.
Essential Functions
  • Lead general external recruitment efforts for most positions.
  • Accurately administer and maintain the applicant tracking system (ATS), including managing candidate activity and status updates.
  • Assist with external job posting management, including posting, updating, and removing job advertisements across approved platforms.
  • Conduct phone screenings to assess candidate qualifications, availability, and interest.
  • Prepare concise phone screen summaries and present recommendations to hiring managers and Human Resource Business Partners (HRBP).
  • Coordinate and schedule interviews with candidates and HRPBs.
  • Coordinate background checks and pre-employment processing, ensuring timely completion and compliance.
  • Support new hire orientation and logistics, including scheduling, communication, and materials preparation.
  • Manage and process onboarding paperwork and pre-hire documentation.
  • Coordinate and support internship and summer hire programs, including recruiting, onboarding, and tracking.
  • Assist with and attend job fairs and other recruiting events as needed.
  • Serve as a point of contact for candidates throughout the recruitment and pre-employment process.
  • Manages the Valley Queen housing program.

Position Overview

Hours: Monday - Friday daytime hours, full-time.

Essential Job Functions:

- Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
- Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress and expected outcomes.
- Delivery of occupational therapy services to comply with prescribed physician orders.
- Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
- Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
- Participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc.
- Documents according to policy and procedure and requires minimal supervision or instruction.
- Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers
- Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
- Supervises other personnel (OTA, HHA) as applicable.
- Completes OASIS assessments where allowed by state professional practice and regulation.

Requirements:

- Degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
