Responsibilities:About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
- Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
- Administer and order point of care and lab-based testing as needed
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
- Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

- Consistency – Although we conduct assessments Sunday – Saturday from 8 am – 7 pm, our full-time (FT) providers typically work Monday – Friday from 8:00 to 5:30.
- Competitive Pay – Earn an annual base salary ranging from $95,500 - $158,400 based on market compensation analysis and experience.
- Incentive Program – Eligible for FT Clinician Incentive Plan in support of leaving no patient behind which includes monthly (uncapped) and year end (up to $25K) incentive opportunities. In 2025, the average payout from this plan was $1,500 per month and $7,500 at year end, totaling $25,500 annually.

Skills & Experience That Shine
- Master’s Degree OR commensurate experience and satisfactory completion of NP or PA licensure
- Current NP/PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- NCCPA, AANP, AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP or PA experience preferred, new grads encouraged to apply!
- Able to travel up to 15% of the time (including overnight travel) to additional locations as needed
- Valid state driver's license for the state of residence

The Certified Nursing Assistant (CNA), under the direction and supervision of the Director of Nursing and/or the assigned charge nurse, provides individualized care to residents to include routine daily care, promoting maximum independence, safety, and comfort for each resident.
*Full Time, Part Time Plus and Part Time options available.
Starting hourly rate is $19.00/hour and increases with experience.
ESSENTIAL JOB FUNCTIONS
- Makes rounds to resident rooms at least every two hours to ensure resident needs are met to include toileting, turning, repositioning, and hydration; makes walking rounds at shift change with the oncoming CNA ensuring resident rooms are tidy and in good condition; communicates any pertinent changes or things that are occurring with the residents during walking rounds ensuring a seamless transition of care; checks the care base, country kitchen, and soiled and clean utility rooms during walking rounds for tidiness; issues found during rounds will be addressed by the off-going shift
- Observes unusual symptoms or signs of illness or skin breakdown and reports same to nurse in charge; reports indications of residents deteriorating physical, emotional, or mental health through the utilization of the Interact Stop and Watch documents
- Listens to change of shift report and communicates with charge nurse on what shift priorities should be
- Provides care in accordance with the resident care profile and resident preferences
- Administers bed baths daily and showers twice per week unless resident requests otherwise
- Distributes and sets up resident meal trays; assists the residents who need assistance with their meals, encourage intake, and accurately document intake
- Provides daily and weekly personal care including oral hygiene, fingernail trimming, dressing daily and throughout the day as needed, assisting with toileting or changing throughout the day as needed
- Positions residents properly using comfort and safety devices as ordered; repositions bedridden residents every two hours or more often as directed
- Takes vital signs according to resident plan of care or as assigned by supervising charge nurse
- Assists with bladder and bowel training of selected incontinent residents
- Makes rounds of incontinent residents every two hours to provide partial bathing and change of clothing and linen as needed; cleans carpet or furniture if needed and notifies Housekeeping for sanitation
- Answers resident call lights in a timely manner, politely, and with focused concern on the resident; maintains a goal of answering each call light in under four minutes
- Answers the phones located at the care base or nurses station when appropriate; provides professional and courteous direction to the party on the phone
- Assists with the admission and discharge of resident and post mortem care
- Completes the shift responsibility checklist for the specific shift assigned
- Assists with weighing of residents on admission, once a month, and as ordered; if weight is greater than set parameters, reports weight change to the nurse
- Assists residents in the use of adaptive equipment including splints, adaptive plates, bowls, cups and utensils, reachers, sliding boards, walkers and wheelchairs
- Appropriately addresses all allegations or sightings of potential abuse/neglect by rescuing the resident, reporting it immediately and reassuring the resident
- Empties urinary drainage bags twice per shift, and totals output for nurse in charge; passes, empties, and sanitizes bedpans and urinals as needed
- Makes sure residents clothing is clean, puts soiled clothing in hamper and puts away clean laundry
- Assures overall neatness and cleanliness of residents room, bedside equipment, and bathroom; makes beds and returns clothing and equipment to proper place
- Participates in mandatory in-services/drills
- Serves an active role in actual Emergency Operations scenarios to ensure the safety of the residents
- Assists with transportation of residents to and from activities; involves residents in daily activities when time allows; engages residents in one on one activities as part of daily routine
- Assists with moving a resident from one room to another including bed/mattress and personal belongings
- Accurately documents care provided to assigned residents using MyUnity Point of Care daily prior to leaving for the day
- Performs all work in a safe manner; reports any safety hazards to the Charge Nurse immediately including improper disposal of medications and sharps; uses safe lifting and transfer techniques and proper body mechanics at all times; complies with emergency drills; maintains infection control standards
- Reports damaged or unsafe equipment to the Charge Nurse while discontinuing the utilization of the piece of equipment; clearly labels the equipment as faulty so that no other employee uses it
- Maintains a positive attitude to ensure dignity and quality of life for residents, and maintains positive interaction with guests and coworkers
- Serves as a positive mentor for new staff members, training them in the proper policies and procedures to ensure quality of care

The Registered Nurse (RN) ensures the delivery of quality care by following the nursing care plan and supervising Licensed Practical Nurses, Certified Nursing Assistants, and orientees. - Perform nursing assessments on all patients within 8 hours of admission.

- Identify teaching needs and goals based on nursing assessment and collaborate with patients and the interdisciplinary team.

- Document all assessments, interventions, and patient responses in accordance with policy.

- Participate in the patient rehabilitation program and manage direct patient care using the nursing process.

- Initiate discharge planning and patient education from the day of admission.
