As the Population Health Clinical Pharmacist, you will provide Population Health Management Services to patients of the Medical Group of ChristianaCare. The Clinical Pharmacist is responsible for completing comprehensive medication reviews for eligible patients, as well as ensuring that performance targets are met or exceeded for medication-related measures across value-based contracts. Primary Function:
