Patient Care Type: This unit is designed to provide transitional care for adult and geriatric patients who require mechanical ventilation (invasive through a tracheostomy or non-invasive ventilation) or have complex respiratory care needs requiring complex respiratory monitoring.

Nursing Team Roles: The focus of care is grounded in the primary value of Mayo Clinic, in that the needs of the patient come first. Nursing provides care in collaboration with the multidisciplinary team and engages the patient and family through participation in structured/scheduled family meetings, to enhance the patient experience and provide a congruent plan of care. The unit is staffed by a multidisciplinary team composed of members from respiratory therapy, physical therapy, occupational therapy, pharmacy, nursing, social work, and nutrition. Providers consist of Pulmonary/Critical Care Fellows, Critical Care Nurse Practitioners/Physician Assistants, and Critical Care Physicians from the Department of Medicine, Division of Pulmonary and Critical Care. 
 

The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. A subset of employees may be required to drive their personal vehicle as a part of the responsibility of their role.

Qualifications

All entry-level associate degree registered nurses with a RN start date of April 1, 2020, and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within five years from the last day of the month of the RN start date.

1 year professional RN experience required

Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. We have an opportunity available for a regional class A driver that will work Monday through Friday. It will be paid by the mile and will require the employee to drive a 10-speed tractor with a short sleeper bunk within the full guidelines of the FMCSA. What You’ll Do

  • Provide podiatric care to residents in long-term and senior care facilities
  • Assess, diagnose, and treat foot and lower limb conditions
  • Deliver preventive care for high-risk patients, including diabetic foot management
  • Educate patients and caregivers on mobility and foot health
  • Collaborate with facility staff for patient-centered outcomes
  • Participate in Medicare quality programs to ensure top-tier care

What We’re Looking For

  • Doctor of Podiatric Medicine (DPM) degree (required)
  • Active state licensure (or eligibility to obtain)
  • Experience in long-term care/nursing home settings (preferred)
  • BLS certification required; ACLS preferred
  • Ability to thrive in fast-paced, independent settings
  • Able to lift/carry up to 15 lbs. as needed



Compensation

  • Providers are compensated based on a percentage of the revenue they generate through billing and collections. Based on 50 working weeks per year, estimated annual earnings range from $20,000–$70,000 per day per week worked (e.g., 1 day/week = $20,000–$70,000 annually; up to 5 days/week = $100,000–$350,000 annually). Actual earnings vary based on caseload, patient volume, services rendered, and billing and collection performance. A comprehensive overview of our compensation structure will be provided during the interview process.

Responsibilities:

As the director, you will independently manage and lead this branch, along with unparalleled and ongoing corporate training and support.

Business Development: Develop and execute a marketing plan to establish Cornerstone as the preferred in-home care provider.

Referral building: Cultivate and manage relationships with referral partners (hospitals, hospices, senior living, etc.).

Staffing and Scheduling: Hiring and onboarding new team members and ensuring all shifts are properly staffed. Monitor on-call responsibilities as calls come from clients, caregivers and partners.

Financial Management: Oversee office budget that is reflected in profit and loss statements.
