Overview
Our team approach supports new and experienced nurses. We guide and encourage patients before, during and after their course of treatment. We triage our patients over the phone and in person. We work closely with radiation and surgical oncology, nutrition, and palliative medicine to coordinate care for our patients. We find solutions to patient problems by working closely with our Oncologists. Most importantly, we build lasting relationships with our patients by supporting our patients through the most intimate and vulnerable times of their life.

Responsibilities

Clinical Excellence/Patient Care
- Provides evaluation, counseling and therapeutic services to chemically dependent patients/families assessing patient’s social, psychological, emotional, financial, cultural problems and chemical abuse history.
- Makes recommendations for treatment and acts as a liaison and patient/family advocate with the treatment network.
- Maintains open communication with referral sources, families, or others involved in the patient’s treatment regarding the patient’s progress, discharge plans and aftercare plans.
- Supports the patient and family throughout treatment by maintaining open communication with referral resources, patient and family. Assists the patient and family in working towards positive outcomes.
- Coordinates the patient’s care with the interdisciplinary treatment team.
- Develops individual treatment plans based upon information gathered.
- Facilitates programming for court ordered referrals.
- Assists patient and family with adjustment to illness, motivation for treatment and realistic planning for post treatment care. Coordinates the completion of the Aftercare Plan with patient and recommends post-treatment referrals for patients.

- Maintains current knowledge of behavioral managed care and the precertification and utilization review activities necessary to obtain treatment approval.
- Participates in covering weekend inpatient evaluations on a rotating basis.

The Clinical Resource Unit (Float pool) is committed to our Nursing Professional Practice Model; we believe in caring relationships with our patients, families, community, and colleagues to provide exceptional care. Compensation and Benefits
The compensation range for this position is $31.37per hour - $53.96per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model. The Carle BroMenn Cath Lab performs many exciting and innovative procedures including but not limited to: diagnostic and interventional cardiac procedures, electrophysiology, peripheral vascular, neuro interventional, and interventional radiology procedures. The Cath/IR RT produces quality imaging and assists the Interventionalist in performing invasive procedures; as well as technical hemodynamic monitoring and interpretation using automated techniques. Providing care to meet patient needs and handle other duties as required and assigned.

Advanced Cardiac Life Support (ACLS) within 1 year - American Heart Association (AHA); Registered in Radiography (R) - American Registry of Radiologic Technologists (ARRT); Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Radiologic Technologist - Radiography - Illinois Emergency Management Agency (IEMA)
