As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, you will work to help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
Primary Responsibilities:

* Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care
* Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other members of the health care team
* Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician
* Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
* Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals
* Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy
* Completes all patient evaluations and develops the PT plan of care within state specific guidelines
* Reports outcomes of evaluation, goals, and anticipated projected frequency of care

Required Qualifications:

* Current Physical Therapy licensure in state of practice

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).   • Reviews service contract booking packages. Includes completing customer number request forms, reviewing and completing Installed Base forms and verifying the service contract transmittal forms. Creates branch service file and mechanic file.

  • Receives and reviews final acceptances from construction and modernization departments. Processes owner and unit changes as necessary.
  • Prints special deck invoices and mails to customers. Prints service contract invoices upon request from customers or branch.
  • Participates in monthly A/R conference calls with Regional Collectors. Actively pursues and follows-up on A/R items.
  • Prepares and submits credit memos. Actively cleanses unapplied credits and submits washout requests as necessary.

Our dedicated team of Respiratory Therapists delivers a wide range of essential therapeutic services, including administering medical gases, performing arterial blood gas analysis, managing airways, and conducting pulmonary function tests. Position Overview


Within established organizational and departmental policies and procedures, the Manager of Respiratory Care is responsible for the planning, organizing and directing the overall activities of the Pulmonary functions with a total operating budget of $4.3 million and responsible for managing 32 FTE’s.


Key job functions include achieving quality and budget goals, assuring compliance with JCAHO and CAP standards, and regulatory agency guidelines as outlined by OSHA, CDC and DPH. A commitment to patient safety, customer satisfaction, and efficient/effective use of resources, teamwork, innovation, and performance improvement is required.


Qualifications/Requirements


  • Education/Training: Graduate of an Accredited School of Respiratory Care; Bachelor’s Degree preferred
  • Licensure: Current MA State Respiratory License
  • Certifications: Registered Respiratory Therapist (RRT) Basic Life Support, Advanced Cardiac Life Support, NRP preferred
  • Previous Work Experience: Minimum 5 years managerial experience preferred. Strong background in Acute Care, Critical Care, Pulmonary Function Testing and Polysomnography. Knowledge of Neonatal Respiratory Care preferred.
  • Other: Ability to travel between 3 sites as needed. 24/7 on call availability.
