Minimum Requirements:

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA
• Active State License is Required to Start the Position
• BLS Certification May Be Required from AHA or ARC
• Experience in Home Health is required
• Experience with HCHB/Oasis is preferred
• Ability to manage a productivity expectation of 30 points/week
• Experience with adult and geriatric populations is preferred
