This job assesses patients and safely performs routine radiographic procedures in accordance with prescribed safety procedures and protocols. Serves as a clinical mentor for junior staff and students.

Job Duties

* Verifies physician order and procedure to ensure accuracy.
* Uses radiology information systems to facilitate care.
* Explains procedure to patient to ensure understanding.
* Independently operates equipment to complete imaging procedure according to protocol (may include placement of nasogastric tubes, IVs, urinary catheters, and administration of contrast).
* Reviews patient images prior to transmission to ensure images meet diagnostic quality standards.
* Completes timely quality control procedures in accordance with regulatory standards and performing notification/remediation as warranted.
* Assists in the facilitation of departmental operations (inventory, staff schedules, equipment maintenance, etc.).
* Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Primary Responsibilities:

* Clinical Competence

* Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
* Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
* Provides required supervisory visits

* Documentation and Care Delivery

* Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members
* Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
* Documents patient visits per policy and payer requirements, and syncs timely per LHC policy

* Quality

* Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation
* Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians
* Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence

* Teamwork

* Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning
* Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students
* Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes
* Participates in on-call and weekend rotation as needed to meet patient needs
* Adheres to and participates in the agency's utilization management model

Required Qualifications:

* Current and unrestricted RN licensure in state of practice
* 1+ years of Home Health experience

State Specific Requirements:
LA:
* 1+ years of clinical experience as a Registered Nurse
* 1+ years of clinical experience as a Registered Nurse may be waived for a Registered Nurse with recent clinical experience as an LPN
* RN licensure must have no restrictions

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends.

Primary Responsibilities:

* Able to triage patient needs over the phone until a nurse can provide an in-person visit
* Responds appropriately and with urgency to crisis calls from patients and families afterhours
* Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned
* Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits
* Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days
* Documents problems, appropriate goals, interventions, and patient/family response to hospice care
* Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:

* Current unrestricted RN licensure in the state of practice experience
* Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

State Specific Requirements:

* LA: a minimum of two years full-time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse

Preferred Qualifications:

* Current CPR certification or ability to complete within 90 days of hire
* 1+ years of clinical experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). This job provides respiratory care treatment, oxygen services, diagnostic testing and ventilation support services to both in-patients and out-patients ranging in age from neonatal to geriatric. Monitors and weans Oxygen Therapy and Artificial ventilation, draws, analyzes and reports Arterial Blood Gases and EKGs, and delivers intermittent therapy. Assists with changing of artificial airways, and provides services as ordered by a physician and in accordance with established policies and procedures.
