The Nursing Departments invite dedicated and experienced faculty who are interested in Online Part-time Lecturer (PTL) opportunities at the undergraduate and graduate levels for didactic and practicum assignments.   • The identification of appropriate candidates for home infusion &/or home health care 

  • The identification of the patients’ and/or caregivers ability to provide care 

  • Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home. 

  • Medication connection at the hospital / facility in order to expedite the discharge into the home setting, as appropriate 

  • Coordinates and communicates all discharge planning activities for new patient referrals with Mount Carmel Infusion Services personnel and referral sources. 

  • Appropriately documents the coordination of care activity including, but not limited to: 

  • Home infusion &/or nursing services utilized 

  • All communication with patients, referral sources, physicians and Mount Carmel Infusion Services personnel 

  • All patient &/or caregiver education & training 

  • Interacts with healthcare team members to answer any questions related to home infusion and community-based services as they plan for patient discharges from the acute care setting. 

  • Functions as a liaison between nursing and other disciplines and departments in resolving patient issues. 

  

Minimum Qualifications: 

  • Education: Bachelor’s degree in nursing (BSN) or Registered Nurse (RN) from an accredited college or school of nursing with equivalent experience required 

  • License / Certification: Current license to practice as a Registered Nurse in the State of Ohio required. 

  • Current Certified Registered Nurse Infusion preferred 

  • (CRNI) credential preferred or be willing to obtain within 6 months of hire 

  • Must have valid and current Ohio Driver’s license and reliable transportation to and from work site 

  • BLS Healthcare provider training required 

  • Experience: Must have a minimum of two (2) years' experience in infusion care setting and experience with processing referrals for homecare and pre-certification.
