PERSONAL CARE ASSISTANT/CNA
Salary: $18 - $24 an hour, based on years of relevant experience.
The Personal Care Assistant is responsible for providing and assisting Participants with direct and indirect care needs, including functional and instrumental activities of daily living in the community. The Personal Care Assistant will perform care for Participants primarily within their residence and occasionally within the LIFE Pittsburgh Adult Day Health Centers. Will provide direct care services, as approved and identified on the Participants Plan of Care to include, but not limited to the following ADLs: bathing, dressing, toileting, eating, mobility, van assist and medication prompting. Will also provide direct care services, as approved and identified on the Participants Plan of Care to include, but not limited to the following IADLs: light housekeeping, meal preparation, laundry, grocery shopping and companionship. The Personal Care Assistant will provides approved basic nursing care services to include, but not limited to, Foley catheter care, applying assistive devices, and assisting with colostomy bags consistent with policies and procedures. And is also responsible for completing and submitting accurate and timely documentation including Flow Sheets, Participant Logs, Communication books, etc. within the appropriate timeframe.
LOCATION/TRAVEL REQUIREMENTS:
Positions are based out of an Adult Day Health Center, which are located in North Shore, Allegheny Center, Greentree, and McKees Rocks. Services areas vary based on location.
Travel to multiple Participants residences throughout a shift is required.

Responsibilities:About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
- Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
- Administer and order point of care and lab-based testing as needed
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
- Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

Responsibilities:
Our Hygienists are responsible for delivering direct dental care and associated services to patients under the general supervision of a dentist and for providing general support to the office.
- Deliver quality and compassionate care to every patient
- Deliver direct patient care to patients, including scaling and prophylaxis, x-rays, application of sealants and fluoride, and related procedures, under the general supervision of a dentist
- Provide excellent customer service by looking for opportunities to offer Same Day Care
- Educate patients and parents in oral hygiene instruction
- Assess the dental condition and needs of patients and review patients' medical history
- Responsible for adhering to all government regulations and company standards
- Guide parents and patients through our entire treatment process
- Assist with dental and perio charting; when necessary
- Take x-rays and save into patient charts, when necessary
- Provide memorable patient experiences by showing compassion for our patients and parents

Requirements:
- Current licensure as a Dental Hygienist in good standing
- CPR (BLS) from the ARC or AHA, including hands-on skill assessment (required upon hire)
