Clinical Excellence/Patient Care

• Provides evaluation, counseling and therapeutic services to chemically dependent patients/families assessing patient's social, psychological, emotional, financial, cultural problems and chemical abuse history.
• Makes recommendations for treatment and acts as a liaison and patient/family advocate with the treatment network.
• Maintains open communication with referral sources, families, or others involved in the patient's treatment regarding the patient's progress, discharge plans and aftercare plans.
• Supports the patient and family throughout treatment by maintaining open communication with referral resources, patient and family. Assists the patient and family in working towards positive outcomes.
• Coordinates the patient's care with the interdisciplinary treatment team.
• Develops individual treatment plans based upon information gathered.

• Participates in conjoint conferences with patients, concerned persons and referral sources as appropriate.
• Develops, promotes and maintains a good relationship with self-help programs such as AA, NA and Al-Anon.
• Participates in mandatory education and department staff meetings, per established guidelines.

Provides assessments for patients with diagnostic impression and treatment recommendations. Supports patients who are seeking help to become abstinent from substances. Teaches patients what the disease of addiction is, and it's signs and symptoms. Helps patients identify resources in their community which support ongoing abstinence. Teaches family members and loved ones about addiction disease and what their part is in the recovery process.

-Positions patients correctly, using proper immobilization techniques-Performs optimal scans using correct technical factors, shielding and protocols, based on the patient diagnosis, history and other relevant clinical information.-Use correct technical factors in accordance with the American College of Radiology (ACR) to use the lowest amount of radiation to get a diagnostic scan.-Performs CT angiography, 3D, MIPS, VR, Vessel Analysis, and MPR's, post processing of computer data to enhance exams for radiologist interpretation, using reconstruction software appropriate to the pathology or region of interest.-Follows correct CT Department Technologist workflow ( select correct scan room, identifies and documents two forms of identification on each patient, acquire history, verify orders, scan patient, send images to PACS, scan all paperwork and data sheets (eg. history sheet, med-reconciliation sheets), complete the case in Meditech, address Q A issues, charge for contrast media and supplies, Verify that all images, series and data sheets have been sent to PACS make sure the number of images is correct, verify exams with images and archive images on scanner-Operates support equipment for CT such as: Independent workstation, pressure injector, PACS and EPIC systems.-Initiates IV for administration of contrast media for CT in accordance with department policy and utilizing standard precautions.-Prepares patient for exams by explaining procedure and by encouraging questions and answering any questions the patient may have-Transports patients to and from departments when necessary. Assist in transfer of patients on and off the CT table. Responsibilities
Serves as a consultant when difficulties with lactation are beyond the knowledge and/or training of the health care provider. Consults, develops and implements lactation plans of care. Collects data, identifies, monitors and evaluates quality and safety issues related to practice. Maintains phone consultation system for lactation questions and problems. Represents the lactation department at unit/system/community meetings. Evaluates and integrates research findings into lactation practice. Maintains current knowledge of lactation research and reflects this knowledge in current lactation practice. Participates in lactation research projects. Pursues continuing education relevant to lactation consultant practice including reading current professional journals, participating in workshops, seminars, in-service programs, conferences, accredited courses, and professional organizations. Directly supervises the breastfeeding patient in the nursing units as required. Instructs in proper hold, latch and breastfeeding positions. Instructs new nursing staff in breastfeeding techniques. Communicates to OB or Pediatrician relating to specific abnormal assessment as seen. Documents clearly findings and required plan for the nursing dyad in EPIC.

Overview

Under the direction of a Registered Nurse (RN), the Health Care Technician (HCT) functions as a support to the licensed nursing staff and performs activities related to the personal care and hygiene needs of an assigned patient group. Facilitates communication with other departments, visitors, patients, medical staff and employees. Serves as a public relations liaison for the unit.

Responsibilities

Assists with the maintenance of unit supplies to ensure the smooth function of the unit, including stocking nurse servers, cleaning instruments and equipment, ordering supplies, and making up charts. Collects data on assigned patients as directed by Registered Nurse (RN) and ensures accurate documentation. Demonstrates competency in selected psychomotor skills as outlined in the skills checklist including: measurement of vital signs, blood glucose monitoring, and measuring and recording intake and output. Performs or assists patients with the activities of daily living. Prepares patients for transportation and/or transport. Works closely with nursing to ensure high quality care for the most critical adult and pediatric patients Assists with communication - updating white boards and doing meaningful hourly rounding Floats within the hospital to perform duties as needed Can be utilized as a sitter for patients with that need Assists in setting up any bedside procedure.

The compensation range for this position is $17.89per hour - $28.98per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health.
