What You Will Do
- Manages and provides the full scope of advanced nursing practice for targeted patient populations, which may include short-term treatment of members at the point of care. Evaluates patient responses to therapy/interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.
- Serves as the clinical authority on individualized care teams and leads rounds and interdisciplinary care team meetings.
- Manages and provides comprehensive, advanced nursing care, including post-discharge aftercare visits, annual comprehensive assessment visits, palliative care-care management program onboarding, and change in condition assessment visits (including physical examination, comprehensive history, screening for physical and/or psychological conditions, and point of care testing). Urgent interventions (i.e., escalations for the Care Teams, RPM, and the 24/7 Line), pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling, and patient education. Establishes a treatment plan based on clinical findings and determines when further evaluation by the collaborating physician, specialist, or emergency care is warranted.
- Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings.
- Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements.
- Manages inter-professional team efforts regarding the medical, nursing, therapy and ancillary care provided to patients to ensure quality outcomes are achieved.
- Participates in on call coverage schedule to ensure 24/7 access to practice clinicians.
- Assesses, educates, and improves client and caregiver knowledge of chronic disease, self-care management, and identification of changes in health status, including appropriate responses and actions through individualized education and inter-professional interventions.
- Observes and analyzes team performance patterns related to population under care and assists in developing interventions to improve team performance. Provides remedial support and guidance to interdisciplinary team members to address implementation/evaluation of plans that maintain/increase customer experience with care, cost-efficiency, and quality care compliance with regulatory standards.
- Communicates with internal and external care partners regarding the needs of the patient or population to ensure interventions occur in a timely and appropriate manner. Intervenes as needed when the care plan is not executed and remediates the situation to prevent reoccurrence.
- Performs procedures as outlined in collaborative practice agreement and as privileged by the Credentialing Committee.
- Manages quality of medical record documentation and submits billing information in accordance with Professional Corporation policy.
- Provides care in one or more care settings based on the clinical requirements: virtually, telephonically or travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options.
- Upon request, performs peer reviews of work performance, including quality of care, clinical documentation, coding and billing practices, communication skills, and population surveillance.
- May participate in the VNS Health Medical Care at Home Quality and Professional Advisory Committee and any associated workgroups related to development of evidence based clinical models of care, peer education and training, quality improvement, medical record configuration and reporting, maximizing the use of clinical decision support systems.
- May act as a preceptor for student Nurse Practitioners, providing excellent role modeling of community-based primary care practice.
- Assumes responsibility for continued professional growth, and maintains professional certification and licensure.
- Participates in special projects and performs other duties as assigned.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required
Valid driver's license, as determined by operational/regional needs may be required
Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required
Maintains NPI, Medicaid and Medicare provider numbers preferred
Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred
Must be certified by ANCC or another accrediting Nurse Practitioner body – in order to bill Medicare and meet credentialing requirements required
Education:
Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required
Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required
Work Experience:
Minimum of two years of experience as a nurse practitioner utilizing full scope of practice preferred
Clinical home care experience or two years managerial experience preferred
Demonstrated knowledge of HEDIS and QARR quality measures, ICD-10 and CPT coding for reimbursement of services required

POSITION SUMMARY
The Legal Practice Assistant, working under the supervision of the Office Manager, provides a wide range of administrative, clerical, and document services to support the workflow and client service needs of attorneys and other timekeepers. Job duties require close collaboration and working with other assistants to and lawyers in the performance of a variety of administrative and practice specific duties as assigned. The Legal Practice Assistant is a non‐exempt position. The hours are 8:45 am – 5:15 pm, Monday ‐ Friday and additional hours as required.
ESSENTIAL JOB FUNCTIONS
- Collaborates with firm assistants and other teams to provide designated and "as needed" administrative services to assist in delivering exemplary service.
- Creates, edits, formats, and proofreads a wide variety of documents, including memoranda, correspondence, legal, and court documents.
- Receive assignments from attorneys and others, performing tasks as requested; update requestor regarding receipt and status of tasks/projects, and consistently meet agreed upon deadlines.
- Input attorney time into the WebTime time entry program. Stay up to date regarding client-specific billing requirements. Assist timekeepers with maintaining appropriate time entry requirements and standards through outreach and communication.
- Work with the Billing department in all aspects of the billing cycle, including but not limited to the review of client billing for accuracy, edits to content/grammar; resolution of billing problems; and efficient changes to the process.
- Accurately prepare and input attorney expense reimbursements and process client-related vendor invoices for payment.
- Sort, open, prioritize and route incoming mail. Prepare outgoing mail including U.S.P.S., U.P.S. and Federal Express. Coordinate messenger pick-ups and deliveries when necessary.
- Coordinate meeting set-ups, including but not limited to conference calls, Zoom and Teams meetings and in-person meetings.
- May assist with the organization and planning of events. Assists with collection of marketing materials.
- Coordinating with attorney talent team, may obtains approvals and coordinate attendance for outside training programs (MCLE).
- Assists with creating and maintaining client/matter and communication lists.
- Maintain calendar and contacts for attorneys. Maintain electronic filing system via Outlook.
- Perform document conversions, e.g., PDF to Word, etc.
- Handle telephone calls from internal and external sources including taking accurate messages and appropriately routing calls.
- Coordinate prints, scans and duplicating projects with Digital Document Center or Print/Mail Service Centers.
- Coordinate travel arrangements (air, hotel, rail, car rental and black car) with firm-provided travel agency.
- Coordinate the New Matter Intake process. Prepare, finalize, and distribute new client Engagement Letters, New Business Memo, etc. Including follow-up with client to ensure receipt of fully executed Engagement Letter. Initiate Conflicts Checks.
- Perform other related duties, special projects, and assignments as required.
- Provide back-up support and vacation coverage.
- Proactively follows-up with managers and colleagues for feedback on projects completed and requests for additional assignments.

The Medical Receptionsit provides reception and clerical services under the supervision of the practice manager and physician(s) for the patients and visitors of the practice. Responsibilties:A day in the life of a Medical Receptionsit at Hackensack Meridian Health includes:
- Receives and directs all communication courteously and consistently. Adheres to HIPAA guidelines and office policy. Phone calls are answered quickly and directed appropriately.
- Greets patients and visitors in a prompt, courteous and helpful manner. Conducts oneself in a friendly manner, by smiling, making direct eye contact and making patients and visitors feel welcome and important.
- Assists patients with the sign in process. Responds promptly to patient's needs.
- Supports financial responsibilities, including: staying abreast of reimbursement and billing procedures, accurately verifies information on Audit Journal, prepares bank deposits and credit card batches and signs and dates completed daily cash receipts.
- Monitors general office supply inventory and equipment, including working with Team Members to determine their supply needs. Notifies Practice Manager, or designee, when supplies are low and initiates ordering of needed supplies and equipment.
- Manages patient check-in process. Consistently and courteously obtains and verifies required demographic information (date stamp and/or copy insurance card). Collects copayment and any applicable balances; Verifies insurance eligibility; Ensures appropriate referrals are obtained (specialty offices); Ensures patient completes appropriate paperwork including but not limited to face sheet, ABN, and HIPAA forms; Accurately enters patient demographic information in billing/computer system.
- Manages patient check-out process, ensuring the process is completed accurately and timely and in a friendly manner. Check-out process includes, but not limited to, reviewing charge slip, posting charges and payments into system, collecting current and past due balance(s), scheduling future appointment(s).
- Promotes patient satisfaction by providing excellent service through friendly and helpful interactions with patients and visitors. Communicates potential patient satisfaction issues to direct manager.
- Maintains efficient and accurate filing and record maintenance system.
- Assembles new patient folders. Retrieves and returns records according to protocol.
- Adapts work schedule to meet the needs of the practice. Works additional hours as requested to maintain adequate coverage for the practice. Willing to cross cover when necessary. Clocks in at designated start time. Managers time effectively to prevent unapproved OT.
- Serves as a pre-loader for our EMR system.

Required Experience and Competencies
- Two to three years of experience in an office or healthcare setting required.

- Act as the point person for all Vituity People Operations (Human Resources) relations to include, but not limited to, ensuring accurate site roster and compliance with employment law and Vituity policies.
- Develop and maintain site orientation checklists and manuals.
- Collaborate with Medical Director and Site Management Team in developing and maintaining site practice policies and procedures.
- As applicable to practice line, responsible for all medical records and data submission to Revenue Cycle Management in a timely manner and responsible for appropriate charge capture in designated system, sending notices and follow up as appropriate.
- Ensure billing and documentation compliance through completion of site WIP/TAD lists, answering provider routine questions, and schedule/hosting/participating in the monthly meeting with Revenue Cycle Management billing team.
- Ensure completion of mandated forms and requests as appropriate to include, but not limited to, death certificates, physician office requests, State specific Workers Compensation and Motor Vehicle Department reporting compliance, and pharmacy requests.
Required Experience and Competencies
- Two to three years of experience in an office or healthcare setting required.
- Associate or Bachelor’s degree in Business Administration, Human Resources or related field strongly preferred.
- Experience working in the healthcare field is preferred.
- Knowledge of healthcare and medical terminology preferred.
- Knowledge of general Human Resource principles preferred.
- Knowledge of Federal, State, and County Agencies who regulate the Healthcare Industry preferred.
- Intermediate to advanced MS Excel, Word, PowerPoint, and Outlook skills.
- Strong consultation skills and the ability to seek out information.
- Strong work ethic, organizational skills, and interpersonal skills.

We are excited to share the base salary range for this position is $23.18 - $28.98, exclusive of fringe benefits or potential bonuses. This position is also eligible to participate in our annual corporate Success Sharing bonus program, which is based on the company’s annual performance.
