Job Summary and QualificationsAs a Physical Therapist, you play a vital role in helping patients regain strength, restore mobility, and rebuild confidence following illness, injury, or surgery. Working closely with physicians and the care team, you'll assess each patient's needs and create personalized therapy plans designed around meaningful goals, active engagement, and exceptional patient outcomes. Your role will include:
- Evaluating patient abilities and developing personalized treatment plans in collaboration with physicians and the care team

- Educating patients and families on exercise programs, adaptive techniques, and safe practices that promote independence

The Registered Nurse, under the direction of the Department Manager, is responsible for professional nursing care and related assistance to adult and geriatric patients. What you will do:
- Participates in assessment and reassessment according to patient’s progress and/or unit protocols. Reports findings and communicates all patient information to the physician.

Job Summary and QualificationsProvides coordinated skilled nursing care to patients of all age groups, in the home. What you will do in this role :
- Assesses home care patients identifying physical, psychosocial and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call-in logs and on-site evaluations.
- Completes OASIS, assessment and visit paperwork according to agency policy. Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members and patient/family/caregiver.
- Communicates significant findings, problems and changes to Clinical Manager and physician, and documents all findings, communications, and appropriate interventions.
- Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided.
- Responsible for participating in on-call rotation and emergency call according to agency policy.
- Graduate of an accredited school of professional nursing
- Minimum of one or more years of home health, public nursing or acute hospital nursing experience
- Familiar with Medicare home health regulations, documentation requirements, ICD-10 coding and PPS (Strongly preferred)
- Reliable transportation and proof of valid automobile liability insurance
- Must have valid driver's license

* Data Lifecycle Management: Manage, manipulate, and clean large-scale datasets. Identify and resolve technical problems or coding errors to ensure data integrity from ingestion to analysis.

Job Summary and QualificationsThis role will float to Cath Lab, EP Lab, OR, Pre-Op and PACU. As a Float Pool Registered Nurse, you will be responsible for performing patient assessments, developing a plan of care, and implementing and evaluating care.
