 Humana is looking for a Field Care Manager, Behavioral Health to join the IL Medicaid team. In this position, you will report to the Manager, Care Management and connect with members both face-to-face and telephonically. The Field Care Manager serves as the primary point of contact, providing integrated care to ensure members receive timely, high-quality, and coordination services that meet their needs. You will employ a variety of strategies, approaches, and techniques to manage a member's health issues and resolve barriers that hinder effective care. Using a holistic, person-centered approach, you will enhance behavioral health outcomes, reduce care gaps and support Illinois' FIDE population through comprehensive, integrated behavioral health care management.

EARN A $3,000 HIRING BONUS! $1,500 is paid after 6 months (180 days) of employment and $1,500 is paid after 1 year (365 days) of employment. You must be employed until those dates to be eligible to receive the payment.

Position Responsibilities:

  • Utilize high-quality, evidence-based behavioral health services through personalized care coordination, crisis intervention, peer support, and strong collaboration with medical and behavioral health providers.
  • Provide comprehensive, integrated support to members experiencing behavioral health conditions, including children, adolescents, adults with serious mental illness (SMI) and serious emotional disturbance (SED), Substance Use Disorders (SUD) and justice-involved members.
  • Engage members in their own communities, meeting them face-to-face whenever possible to build trust and facilitate meaningful care coordination.
  • Coordinate behavioral health and medical services, ensuring appropriate provider engagement and adherence to treatment plans.
  • Improve member's health literacy while simultaneously addressing health related social needs to positively impact member's healthcare outcomes and well-being.
  • Serve as the driver of the member's interdisciplinary care team (ICT), overseeing care planning, transitions, and service delivery.
  • Facilitate ICT meetings, ensuring communication among providers, Service Coordinators, and Care Management Extenders.
  • Proactively support transition of care efforts.
  • Will work with autonomy but reach out when support is needed.
  • Collaborate with internal departments, providers, and community-based organizations to link to appropriate services and create a seamless, culturally competent care experience that respects the members' preferences and needs.
  • Follow processes and procedures to ensure compliance with regulatory requirements by the Illinois Department of Human Services (IDHS), Center for Medicare and Medicaid Services (CMS) and the National Committee on Quality Assurance (NCQA).

Required Qualifications

  • This role is regionally based in Joliet, IL. Must reside in Joliet or a surrounding city (New Lenox, Lockport, Crest Hill, Shorewood, Frankfort, Preston Heights, Rockdale, Fairmont, Ingalls Park)
  • Active Illinois licensed LCSW, LMFT or LCPC (No supervisees or provisional licenses)
  • 2+ years of post-degree clinical experience in behavioral health setting.
  • Case management experience working with complex SMI, SUD, SED population.
  • Ability to travel to region-based facilities and homes for face-to-face assessments.
  • Ability to use a variety of electronic information applications/software programs including electronic medical records.
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
  • Valid driver's license, car insurance, and reliable transportation.

Preferred Qualifications

  • Case Management Certification (CCM)
  • 3+ years of in-home assessment or care coordination experience.
  • Experience working with Medicare, Medicaid and dual-eligible populations
  • Field Case Management Experience
  • Knowledge of community health and social service agencies and additional community resources

Additional Information

  • Workstyle: This is a remote position that will require you to travel.
  • Travel: Up to 75% of the time for collaboration and face-to-face meetings and field interactions with staff, providers, members, and their families.
  • Workdays and Hours: Monday – Friday; 8:00am – 5:00pm Central Standard Time (CST).
  • This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

Interpret and implement medical prescriptions for speech-language pathology services, ensuring appropriateness for the home care environment

Conduct thorough assessments in the patient's home using standardized tools to evaluate communication, cognitive-linguistic function, voice, fluency, and swallowing abilities

Develop and implement individualized, evidence-based treatment plans that reflect both short- and long-term patient goals, with attention to the home environment and available resources

Adjust treatment strategies based on patient progress, caregiver input, and observed response to therapy, ensuring goals remain functional and achievable in the home setting

Document all assessments, interventions, patient progress, and caregiver interactions using electronic or paper records in compliance with agency policies and regulatory standards

Qualifications:

Required Qualifications

  • 6+ months experience in any combination of the following areas: customer service, electrical or utility construction, -OR-technical certification (or higher) -OR- current military service, or prior military service with honorable discharge.

  • Must have and maintain a valid driver’s license and remain eligible for DOT requirements.

  • Upon hire, must successfully pass all components of the Safety Training course curriculum.

Overview

Missouri Baptist Sullivan Hospital is a designated critical access hospital committed to providing quality health care service to residents in Crawford, Franklin and Washington counties.

The hospital has enhanced and expanded the health care services available to the greater Sullivan community with its state-of-the-art emergency department, modern childbirth center, critical care unit, multiroom surgical suite, expanded cardiac rehab services, and a cancer center. A 22,500 square-foot medical office building provides offices for primary care physicians as well as space for a variety of visiting medical and surgical specialists from the St. Louis area.

Missouri Baptist Sullivan Hospital operates four rural health clinics providing quality health care to surrounding communities. In addition, the hospital operates a comprehensive Therapy and Rehabilitation department and an award-winning Wound Care Center with two hyperbaric chambers.

Role Purpose

The Graduate Nurse provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care after completing a nursing program and before licensure. Will be required to obtain licensure within 90 days of graduation from an accredited school of nursing to remain employed in a HSO in the state of Missouri.

You'll Need to Have:

The Maintenance Supervisor ensures equipment, sortation system; conveyors, building requirements, ammonia system and support systems are maintained at the highest efficiency level possible.

What you'll do as a Maintenance Supervisor:
  • Manage budgets for building and repair, maintenance, supplies, third-party support and spare parts inventory.
  • Maintain adequate on-hand inventory supplies to support sortation system. Monitor expenses and purchase actions.
  • Obtain quotes to build strong vendor relationships. Ensure the best cost is being provided.
  • Utilize maintenance connection to optimize asset utilization through scheduled preventative maintenance.
  • Understand all profibus connections and repeaters within the sortation system.
  • Identify, prioritize, and coordinate the daily activities of technicians.
  • Manage the department to achieve desired results in productivity, teamwork, safety and task proficiency.
  • Maintain proper staffing, required training and certifications.
  • Work nights and weekends if critical system failures occur.
  • Ensure the training and development of the team on all technical aspects of DC maintenance including conveyor, HVAC, ammonia refrigeration and building support systems.
  • Monitor expenses by function and equipment type.
  • Provide hands on repairs and preventative maintenance to DC systems as required.
  • Monitor and maintain safety processes to keep the division in compliance with all federal, state and local regulations.
  • Perform other duties assigned

Qualifications you'll bring as a Maintenance Supervisor:
  • Have a 4-year degree or equivalent professional training and experience in an industrial maintenance discipline.
  • Have demonstrated outstanding leadership qualities, including assertiveness, organization, enthusiasm, decisiveness, planning, confidence and accountability.
  • Have strong technical working knowledge of electrical, HVAC, refrigeration, ammonia, energy management, MHE and conveyor systems.
  • Have computer skills including Windows, maintenance database programs and Maintenance Connection.
  • Have a working knowledge of PLC's, high and low voltage systems, including 480V three-phase, and computer controlled automation.
  • Be able to work safely with potentially harmful chemicals, solvents and components.
  • Have ability to work independently and in a team environment.
  • Maintain a flexible work schedule to meet the changing needs of the job.
  • Be able to read and interpret schematics, diagrams, blueprints and CAD. This position requires the ability to read, write, and understand English at a

    level sufficient to perform job-related tasks effectively and safely. This includes understanding work instructions, safety protocols, and communications essential to the role. The requirement is directly related to the nature of the job and ensures compliance with workplace safety and operational standards.
