Job Summary and QualificationsResponsible for providing direct patient care in accordance with physician orders, recognizing standards of practice and establishing company policies and procedures. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

This role is critical in building trust and collaboration between our hospice and the communities we serve. This is a strategic healthcare-facing role perfect for someone with a passion for mission-driven work, relationship-building, and increasing access to hospice care.
Key Responsibilities:
- Serve as a liaison to hospitals, primary care providers, specialists, SNFs, and other potential referral sources.

What you'll do in this role:
Ensure productivity goals of clinical staff are met
Ensure case management guidelines are being followed by each team member
Assign patients to multidisciplinary team and adjust schedules as needed
Serve as the first line for resolution of concerns and issues for the staff

• Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
The Experience and Skills You Bring
- Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
- Must maintain a valid driver's license, good driving record and reliable automobile
- High school graduates or GED preferred
- Demonstrate the ability to be able to work within a multi-disciplinary team setting
- Demonstrate the ability to work in field situations
Preferred Knowledge, Skills, and Experience
- Preferred candidates will have in-home and/or hospice experience
