Role Responsibilities:
- Provide comprehensive assessment, diagnosis, and management of critically ill adult patients, including bedside procedures, physical exams, and diagnostic testing
- Collaborate closely with intensivists, specialists, nurses, respiratory therapists
- Participate in daily multidisciplinary rounds and contribute to care planning
- Ensure high-quality, evidence-based patient care and documentation
Direct Patient Care:
- Perform comprehensive assessment, differential diagnosis, and evidence-based management of critically ill adult patients across the NSCCU and Cardiovascular Surgery ICU (CVICU).
- Independently manage patients with multi-organ system failure, acute respiratory failure, shock states, complex arrhythmia, sepsis, post-operative complications, and advanced hemodynamic instability.
- Perform and interpret bedside diagnostics, including Point of Care Ultrasound, EKGs, hemodynamic monitoring, ventilator waveforms, and radiographic studies.
- Initiate and titrate medications, including vasoactive agents, sedatives, analgesics, antiarrhythmics, insulin infusion, and critical drips according to protocols and patient condition.
- Provide end-of-life support and assist with goals-of-care conversations, in collaboration with attending intensivists.
Procedures (as credentialed)
- Perform ICU procedures, including:
- Central Venous Catheter Placement
- Arterial line placement
- Temporary pacing wire management (CV surgery patients)
- Chest tube/mediastinal drain management and/or removal
- Intubation and airway management
- Bedside ultrasound-guided procedures (paracentesis, thoracentesis, vascular access)
- Swan-Ganz catheter interpretation and management (if applicable for CV surgery ICU)
- Manage advanced devices such as IABP, Impella, CRRT, and ECMO in collaboration with specialists (based on your hospital’s scope).
Night Coverage & Collaborative Decision-Making
- Collaborates with the intensivist to provide overnight care for both NSCCU and CVICU, responding rapidly to changes in patient condition.
- Communicate significant overnight changes to the daytime multidisciplinary team to ensure continuity and safe handoff.
- Triaging with intensivist urgent consults from ED, floors, and post-operative areas for potential ICU admission.
Interdisciplinary Collaboration
- Partner with intensivists, cardiovascular surgeons, anesthesiologists, cardiologists, nurses, respiratory therapists, pharmacists, and dietitians to deliver coordinated, high-quality care.
- Collaborate with CV surgery teams regarding post-operative protocols, including management of chest tubes, pacing wires, anticoagulation, hemodynamics, and early complication recognition.
- Support bedside nurses with real-time clinical decision-making, hemodynamic adjustments, and emergent interventions.
Rounding, Handoff, and Documentation
- Participate in structured evening rounds, ensuring accurate and thorough transitions of care.
- Complete timely, clear, and compliant documentation, procedure notes, critical care time, and billing within the EHR.
- Update families when clinically appropriate and ensure communication of changes to daytime providers.
Quality, Safety, and Protocol Adherence
- Adhere to and help optimize ICU evidence-based protocols, including:
- Ventilator liberation
- Sepsis management
- Glycemic control
- DVT and stress ulcer prophylaxis

- Identify and escalate safety concerns, near misses, or system issues to leadership.

Human Resources Management
- Oversees all human resources functions within the Division(s) and serves as the liaison and contact for all human resources matters. - Manages, supervises and directs assigned administrative/support staff to include determining staffing needs, recruitment, hiring, work assignments, performance evaluation, disciplinary/corrective action and employee training and development.
- Participates in faculty recruitment for the Department which includes developing pro-formas to assess financial/program impact of new faculty and services; recruitment, advertising, hiring and relocation of new faculty, participating in onboarding new faculty; handling and verifying contracts and agreements; working in collaboration with VCU Human Resources in the administration of the annual faculty salary process, and working with the MCVP compensation plan team; and assisting faculty and other providers in the credentialing process and professional liability verification.
- Assists with productivity analysis and accountability of Division faculty.
- Communicates with providers and divisional staff to ensure all remain up to date on current health system policies.
- Works with the Office of the General Counsel and the VCU Office of Immigration Services as needed on visa issues and all related matters for existing/prospective employees from other countries.

The RN / Licensed Practical Nurse (LPN), in collaboration with a Registered Nurse, utilizes the nursing process of assessment, planning, implementation, and evaluation to ensure quality nursing care and to maximize the quality of life for the patients. The LPN is an integral member of the clinical support team, and collaborates with other multidisciplinary members to meet the clinical needs of the patients. The Director of Clinical Services plans, coordinates, implements high quality care through analysis and evaluation of multiple methods and tools that measure quality, such as the Network Performance Measures, USRC Clinical Quality Reports, policies, procedures, and directives. This position assumes a proactive role in adhering to guidelines, policies, procedures, and directives and collaborates with Clinical Specialists, Facility Administrators, and clinic staff to ensure compliance with company policy, Federal and State agencies.
Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.
GROWTH
- Oversight of guidance provided to the facility teams by the Clinical Specialists within assigned regions.
- Work with regional operations management to assist with the opening of denovo facilities and transitioning acquired facilities.
- Evaluate trends in clinical data and provide guidance to Clinical Specialists to prepare for educating the facility staff on the effective and efficient use of company resources, (i.e. supplies), safety and risk reduction, and the best methodologies to achieve.
- Evaluate readmissions, hospitalizations, and catheter rates to identify opportunities to improve treatment rates and decrease infections and missed treatments.
- Work with Clinical Specialists toward the achievement of monthly, quarterly and annual projections based on clinical outcomes and management objectives.
OUTCOMES
- Develop, implement, and monitor quality of care processes for program regulatory compliance in accordance with Company goals.
- Ensure clinical processes in assigned USRC facilities are maintained in accordance to company policy and federal, state and local regulations. Review audits performed by Clinical Specialists in assigned facilities. Ensure findings are reported to the Facility Administrator and regional management teams and POC is developed and implemented appropriately. Report delays in the execution of the POC to the VP of Clinical Services and RVP as indicated.
- Assist with the development of quality goals and measures by reporting field requests and trends in data to the VP of Clinical Services. Responsible for positively impacting clinical outcomes by working collaboratively with the clinical and local management.
- Oversee Clinical Specialists in achieving program target goals for patient outcomes in accordance with quality patient care and Company goals at assigned clinics.
OPERATIONAL READINESS
- Knowledge of and remain current with technology, federal, state, local laws and regulations.
- Knowledge of state specific education requirements. Collaborate with policy development committee with the purpose of developing state specific policies, where indicated.
- Assist Clinical Specialists as needed to support clinics with remaining in compliance with all applicable federal, state, and local laws and regulations by conducting internal clinical audits. JOB PURPOSE/SUMMARY
Summary of role of team:
- "Family Empowerment Initiative" or "FEI" provides support to low-income families, children ages birth (0) to eighteen (18), and their caregivers.
- This includes, but is not limited to, intensive services, crisis services, self-sufficiency through trauma informed services, case coordination, peer support, parent/caregiver education, and family engagement strategies.
Summary of position:
- The Care Coordinator provides trauma-informed, family-centered services to children and youth experiencing a mental health crisis, including those referred from mobile crisis units, crisis stabilization units, walk-in centers, emergency departments, psychiatric hospitals, and other acute care settings.
- The Care Coordinator provides linkage to community resources, clinical services, and natural supports related to the child/youth.
- Engagement in both person, and collateral contacts required. Staff will be an active participant in FTM to create the Individualized Services Plan (ISP).
TYPICAL WORKING CONDITION/ENVIRNMENT
- This position spends the majority of their time working at Knox Children & Youth (C&Y) Center, and seeing individuals in their homes and community.

1. Direct Care
Initial Contact:
- Make initial contact with the referred family within twenty-four (24) hours of receiving the referral.
Initial Visit:
- Provide initial intake in partnership with the Peer Support Specialist visit within 72 hours of referral.
First Month of Services:
- Conduct a minimum of one face-to-face, in-home visit or one telephonic or video contact with the family each week during the first month of services.
Second and Third Months of Services:
- Conduct a minimum of one face-to-face, in-home visit or one (1) telephonic or video contact with the family each week during months two and three.
- Ensure that at least one of these weekly contacts is a face-to-face, in-person visit every other week during months two and three. One face-to-face, in person meeting can be the monthly FTM.
- Complete an Eligibility Assessment and Assessment Plan for each eligible Low-income family within ten business days of initial contact.
2. Quality Care
- Responsible for ensuring all phases and activities of the Wraparound Process are done to the high quality and fidelity - accountable to family team.
- Provide outreach and engagement to families upon referral to the program by conducting initial phone contact and the intake process with the assistance of the Peer Support Specialist (PSS).
- Complete an assessment of the family including Child and Adolescent Needs and Strengths (CANS) assessment, Caregiver Strain Assessment, and other national evaluation tools to develop a care plan.
- In partnership with the PSS, provide face-to-face direct service hours to families including support, referrals and assistance with the development of positive parenting, safety planning, basic life skills and parenting a child/youth with special needs.
- In collaboration with the PSS, assist families in developing a list of individuals they want to attend the Family Team Meetings.
- Ensure that the wraparound process is family driven and that families experience ongoing participation in and ownership of their care plans.
- Provide liaison functions for the team members and service providers.
- In collaboration with the PSS, creation of the family team and scheduling of case conferences/family team meetings in collaboration with PSS and family members.
- As a result of the family team process, develop, and coordinate Individual Service Plan (ISP) with the family and PSS.
- Linkage to community resource, clinical services, and natural supports related to the child/youth.
- Be available for crisis calls; collaborate to provide support during the crisis and following the crisis.

COMPENSATION:
- Starting salary for this position is approximately $19.34 /hr based on relevant experience and education.
Schedule:
- Primary working hours are Monday through Friday, and should be provided at times that meet the needs to the families served.
- On site supervision is preferred, and position must be available via phone if a function of the position requires being out of office.
Travel:
Equipment/Technical Competency:
- Position requires working knowledge of Excel, Word, Microsoft Office, and any other platforms required by funders.
- Position requires regular use of a computer, and appropriate phone etiquette.
- Position is required to respond to all voicemails, emails and messages within 24 hours.
Equipment/Technology:
- Position requires a cell phone, in the event staff need to contact the position when not in the office.
- Cell phone may be provided by the Center or reimbursement for these expenses is considered.
QUALIFICATIONS - FEI Care Coordinator
Experience / Knowledge:
- Must possess a minimum of B.A or B.S. degree in social work, psychology, or related field with a preferred minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with mental health conditions.
- Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
- Effective verbal and written communication skills.
- Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community.
- Ability to develop and deliver case presentations.
- Ability to analyze complex information, and to define and solve problems.
- Ability to work effectively in a team environment.
- Ability to work in partnership with peer/family support specialists with lived experience.
Education / License:
- A minimum of a bachelor's degree in a social or behavioral science or other related acceptable field with one to three years' experience or training in social services.
Physical/Emotional/Social - Skills/Abilities:
- This position requires utilizing a personal, dependable vehicle to conduct Center business.
- Maintaining a dependable vehicle and certified driver status is a condition of employment.
- This position also requires that the employee have an F Endorsement on his/her Driver License to be available to transport individuals as needed.
- Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, running, and walking.
- Physical de-escalation techniques will only be implemented at Helen Ross McNabb Center facilities, and not in community settings.
- Additionally, an essential job function is a reliable, predictable 40 hour per week job, on site and in the community, with regular attendance.
- Hearing of normal and soft tones.

- Valid driver's license.

- Frequent sitting, standing, walking, bending, stooping, and reaching.
