About the job:
Schedule: Monday - Friday, 8:00a - 4:30p, rotating on-call after hours. Fully onsite There are occasional weekends.
Travel: Yes, intermittent local travel to participants' homes is required.
About The Nurse Practitioner role: The NP is under the supervision of the Medical Director provides primary care to participants. Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care to Senior Community Care program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Senior Community Care standards.
Required Qualifications:
- Licensed as Registered Nurse (R.N.) in the state of employment
- Graduate of a school of professional nursing required.
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- A minimum of one year's experience in working with the frail and elderly population required.
- Shall have either training or related experience in the job assigned.
Preferred Qualifications:
- Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred.
- Two (2) years' experience as a nurse practitioner in a geriatric setting desirable.
Essentials:
- Responsible for oversight of residents of VOANS Skilled Nursing Facilities.
- Evaluate residents of VOANS Skilled Nursing Facilities.
- Provide medical care and treatment for residents of VOANS Skilled Nursing Facilities.
- Coordinate care of residents with Primary Care Physicians and Medical Directors of VOANS Skilled Nursing Facilities.
- Performs in person comprehensive history and physical on new Senior Community Care participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in person reassessment semiannually and as needed.
- Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.

The Physical Therapist under the supervision of the Center Manager, provides physical therapy services to program participants. Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.

Essentials:Β
- Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
- Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
- Evaluates participantβs needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
- Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
- Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

- Assign store sweeps, monitor/maintain cash register supply levels and maintain cleanliness standards in the restrooms and parking lots
- Maintain visibility of the exit doors for security and loss prevention issues
- Schedule and oversee proper execution of appropriate meal and rest breaks in compliance with company policy and state requirements
- Assist in the hiring and training of new Cashiers and taking necessary steps for disciplinary actions

To be a Head Cashier at Sprouts Farmers Markets you must:
- Be at least 18 years of age; possess a high school diploma or GED, and 1-2 years of recent cashiering experience supplemented with leadership training.
- Be dependable and reliable having the ability to work a flexible schedule that changes as the business changes; including nights, weekends and holidays.
- Have and show an outgoing and friendly behavior, a positive attitude and the ability to interact with our customers, representing as a role model in behaviors.
- Ability to operate front end equipment; register, calculator, scanner.
- Have good communication skills; the ability to take direction/constructive criticism and participate in a team environment
- Be able to perform repetitious activities, and have the ability to multi-task, prioritize and stay organized.
- Must lift/carry product horizontally/vertically weighing up to 30 lbs., from 36" to 24", for a distance up to 3 feet for up to 50 hours without mechanical assistance.
- Must use both hands to move grocery carts horizontally requiring a force up to 35 lbs., for a distance up to 4 feet for up to 50 hours.
- Requires strong attention to detail and proficiency with numbers; proficient in cash handling and mathematical skills; ability to read, write and count.
