Manager Pharmacy Supply Chain Summary:

• Responsible for oversight of the procurement of medication and medication supplies for the BayCare hospital pharmacies.
• This includes the coordination of the BayCare pharmacy purchasing activities which is comprised of ordering, receiving, and maintaining databases used to track inventories and the scheduling, recruitment, orientation, performance reviews, and disciplinary action of the pharmacy buyers and associated coordinators.
• Oversees management and communication of inventory strategies to ensure adequate stock levels are met.
• Evaluates pharmacy purchasing contracts and assessing and maintaining contract compliance.
• Maintains compliance with regulations associated with the pharmacy supply chain, including DSCSA and recall management.
• Responsible for drug distribution to and from the BayCare Central Pharmacy, including inventory management and dispensing operations.

Minimum Qualifications:

Required Education: Bachelors or PharmD – Pharmacy.

Preferred Education: Masters - Pharmacy, Health Administration or Business.

Required Experience: 1 year Pharmacy and 1 year - Management Role in a related field or

Equivalent experience including 2-year HSPAL or Pharmacy Supply Chain residency program.

Preferred Experience: 3 years supply chain and/or 340b experience.

Required Credential: Pharmacist - State of Florida

Preferred Credential: ASHP (American Society of Health-System Pharmacists) Accredited Residency Program Certificate.

Summary:

All pharmacists fill orders for drugs, monitor patient drug therapies and provide drug information. Pharmacists provide pharmacy services with compassion, excellence and efficiency. In addition, level II pharmacists participate in management of patient pharmacotherapy and quality medication management within assigned service lines.

Responsibilities:

Operational Duties and Responsibilities

• Prepares and dispenses drug orders per physician request according to established policies, procedures and protocols.
• Interprets drug orders (verbal and written) and transcribes/verifies into computerized patient medication record accurately. Maintains, accurate, complete patient drug record.
• Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition products accurately.
• Issues controlled substances to patient care areas and maintains records as required by law and institutional policies.
• Provides services efficiently and in a timely fashion.
• Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians
• Maintains competency required for current job title/position
• Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files. Attends staff meetings.
• Completes all competency/skills assessment requirements.
• Provides analysis of medication use processes and contributes at least one related project per year.
• Ambulatory care Pharmacist
• Provides ambulatory pharmaceutical care services utilizing approved protocols and/or collaborative agreement with other providers, including managing patient drug therapy, anticoagulation management, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Performs duties in the clinical areas assigned such as but not limited to: anticoagulation clinics, intensive medical home, transition of care, Discharge Medication reconciliation, etc.

Clinical Duties and Responsibilities

• Ensures safe, appropriate, cost effective drug therapies for patients according to established policies, procedures, and protocols.
• Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
• Assists with pharmacokinetics consult service and renal drug dosing per hospital protocol.
• Reads, extracts and interprets information in patient medical records accurately.
• Detects and reports suspected adverse drug reactions accurately and in a timely manner.
• Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
• Provides clinical consultation and clarification to practitioners as appropriate.
• Provides accurate, adequate and timely drug information to the hospital’s professional staff.
• Provides drug education to patients and their families per institutional protocol.
• Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned.
• Conducts designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)
• Baseline departmental standards for reporting interventions for clinical productivity as defined by departmental goals.
• Participates in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned
• Attends interdisciplinary rounds when assigned
• Provides services efficiently and in a timely fashion
• Contributes to the advancement of the pharmacy department and medication management of the organization through (minimum):
• Precepting of pharmacy residents and students
• Assessment of new and ongoing pharmacist competencies
• In-service of pharmacy and/or nursing staff (1 per year)
• Participates in departmental or sub-committees as assigned
• Completes 1 clinical project per year

• Potential for exposure to hazardous and toxic substances (including chemotherapeutic, cytotoxic drugs and cleaning solutions), sticks or cuts by needles and other sharp items.

Job Requirements:

Education/Skills

• Doctor of Pharmacy or BS Pharmacy degree required
• One of the following required: BPS Board Certification, Residency, Fellowship or at least 5 years of hospital experience
• ASHP Residency Preceptor status required within 180 days of hire (for Ministries with Residency programs)

Experience

• PGY-1 and PGY-2 residency completion, Board certification, or 5 years hospital experience required at hire

Licenses, Registrations, or Certifications

• Pharmacy state licensure required within 60 days of start date
• Pharmacist preceptor required within 90 days
• BLS or ACLS required
• PALS (for pediatric practice) required

Summary:
The Pharmacy Technician 2 Certified performs duties such as prescription data entry, labeling, preparing and filling of medication orders for patients of the retail pharmacy. Such medication orders are prepared up to the point of final checking and dispensing by registered pharmacists. • Recognizes and identifies various classes of drugs and their generic and trade names.
• Assists pharmacists in coordinating and scheduling preparation of sterile admixtures.
• Performs pharmaceutical math with understanding and accuracy, including: fundamental math functions, common apothecary and metric conversions, pharmaceutical dose calculations, and parenteral dose calculations.
• Familiar with abbreviation symbols and equivalents used in pharmacy communications.
• Understands various drugs' storage requirements, dosage forms and compounding techniques. Demonstrates understanding and proficiency of extemporaneous compounding skills.
• Possesses proficient data entry skills. Demonstrates essential computer skills such as order entry, label generation, drug pricing, and third party billing. Prioritizes workload to maintain an efficient workflow.
• Recognizes various billing plan codes and which drugs are covered under these different plans.
• Assists in looking up information to answer general information questions. Refers questions regarding drug information or patient specific questions to a pharmacist.
• Ensures the I.V. preparation room and related equipment are cleaned, calibrated and maintained per organizational policy and procedure.
• Updates/revises patient inventory usage numbers in the computer based on actual usage, change in orders, and communication with other team members.
• Remains alert to customer needs and provides prompt, courteous customer service.
• Responds to and resolves patient/customer problems in an appropriate and timely fashion using proper channels.
• Maintains work area in an organized and orderly fashion, including facing of shelves and returning medications to shelves.
• Checks and maintains adequate supply of stock necessary to fill prescriptions. Orders stock well in advance of running out. Follows up with stock back orders and processes balance-due prescriptions.

As an Inpatient Pharmacist you will:

• Review and interpret provider orders and dispense medication
• Evaluate drug efficacy and review for contraindications/precautions

• Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required

ChristianaCare is recruiting for a Pharmacy Technician to work full time, day/evening shift rotation. Every other weekend required; holidays as required.

Principal Duties and Responsibilities:

• Assist the pharmacist in providing timely, efficient, cost effective, and appropriate pharmaceutical care to the patient
• Refill patient medication bins through robot cart fill and manual picks; update carts by including new orders, removing discontinued medications, and transferring medications. Update labeling of bins on admission, transfer, and discharge of patients
• Deliver and exchange 24-hour cart fill to nursing units at scheduled times. Check medication storage areas for discontinued and discharged patient medications; ensure that returned medications are placed back into stock
• Reorder medications in short supply and restock the picking station and other areas of the pharmacy
• Send prepared medications via pneumatic tube system. Any medication that cannot be tubed is delivered to the appropriate location on the nursing unit
• Manage automated dispensing cabinet inventory, including accurate selection and loading of medications
• Ensure that medications are stored properly and that only in date medications are stored. Remove discontinued medication
• Receive and check incoming supplier order(s)
• Prepare IV medications using aseptic techniques and in following processes in full compliance with USP 795, 797, and 800 federal regulations
• Use appropriate Personal Protective Equipment (PPE) and closed system transfer devices (as needed) for the preparation of chemotherapy and other hazardous medications

Education and Experience Requirements:

• High School Diploma required
• Pharmacy Technician Certification Board (PTCB) certification required unless enrolled in a PharmD program.
• Applicants with Pharmacy Technician experience are encouraged to apply and will be required to obtain PTCB certification within 1 year of employment.
