Job Summary

The Augmentative & Alternative Communication (AAC) Therapist is responsible for the evaluation, re-evaluation, and the development and implementation of plan of care in accordance with the preferred practice standards/clinical guidelines for children who require Augmentative and Alternative Communication systems. Skilled in the utilization of various speech-generated devices and technology to evaluate and treat complex communication disorders. Participates as an interdisciplinary/multidisciplinary team member in the provision of care and in tracking clinical and quality of life outcomes. The AAC therapist will provide consultation, education, and in-services, as needed in order to improve outcomes for children.

Job Specific Duties

  • Participates in interdisciplinary/multidisciplinary meetings and other related meetings that pertain to the care of specific patients.
  • Maintains and documents productivity standards set by leaders.
  • Records daily patient documentation, statistics, and charges accurately and in a timely manner as determined by meeting departmental standards.
  • Ensures documentation reflects evidence based practices.
  • Accountable to improve access to care for the patients we serve.
  • Conducts and interprets evaluations based on patient's medical concerns, developmental age, diagnosis, and prognosis. Provides recommendations based on findings.
  • Maintains active and supportive communication with the patient, caregiver(s), and others regarding progress, problems, home programs, and other issues related to the therapeutic process.
  • Develops a plan of care outlining area of deficit, activity restrictions, context and assistance needed, and ensures functional, realistic, and measurable treatment goals and/or status of ongoing goals.
  • Recommends and provides treatments based upon medical, clinical, functional, and observational findings.
  • Initiates, documents, and prepares the patient/family for discharge at the time of the evaluation based on maximum potential or when benefit of therapy is reached.
  • Follows TRAAC and initiates, documents & prepares the patient/family for discharge at the time of the evaluation based on maximum potential and/or when benefit of therapy is reached.
  • Provides proof of at least 1 in-service specific to evidenced-based practices/literature review.
  • Provides proof of at least 1 peer review (case study presentation) incorporating evidenced-based practices into their treatment plan.
  • Maintains ongoing CEUs to maintain professional licensure and clinical excellence.

Qualifications

Minimum Job Requirements

  • Master's Degree in Speech-Language Pathology
  • Speech/Language Pathologist (SLP) State of Florida license or Provisional Florida licensure - maintain active and in good standing throughout employment
  • Certification of Clinical Competence by ASHA
  • American Heart Association BLS - maintain active and in good standing throughout employment
  • Finger printing background and Good Moral Character Affidavit must be initiated within 15 days of hire date and finalized within 90 days of hire date

Knowledge, Skills, and Abilities

  • Pediatric experience preferred in Augmentative and Alternative Communication evaluations and treatment of children with complex communication needs.
  • Membership of ASHA SIG related to Augmentative and Alternative Communication highly preferred.
  • Ability to appropriately interact with patients, parents/guardians, vendors, and healthcare professionals in all work-related situations.
  • Ability to communicate effectively both verbally and in writing.
  • Able to maintain confidentiality of sensitive information in accordance with HIPAA regulations.
  • Current in practice standards/clinical guidelines for evaluation, diagnosis, treatment of children with complex communication needs.
  • Ability to support quality outcomes.
  • Familiarity with switches and the corresponding motor and cognitive skills required for effective use of AAC devices.
  • Awareness of how seating and positioning impact an individual’s ability to interact with AAC systems.
  • Proficiency in methods for customizing AAC systems to meet individual needs.
  • Ability to collaborate with vendors to design and deliver training sessions tailored to diverse audiences.
  • Conduct a comprehensive assessment of the individual’s speech, language, and overall communication abilities, taking into account their unique background and communication needs.
  • Awareness of funding sources and the requirements for applying for funding from each source.
  • Analytical ability necessary to evaluate patient condition, to interpret, analyze data, assess patient progress, and determine appropriate follow-up

Primary Function

HSL provides enhanced housing with services to seniors living in its three affordable housing locations, and at partner housing sites, with the goal of improving quality of life and supporting independent living.  At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized case management and support as needed and desired by the resident.

The Nurse Care Manager is a key member of the housing team working to support residents in living independently and safely for as long as possible by developing meaningful relationships with residents and providing supports in a holistic way.  The Nurse Care Manager works closely with other team members to:  engage residents in wellness assessments and health education programs, connect residents to needed services, triage resident issues, provide a wide variety of case management tasks including coordinating care for residents returning home from hospital and/or rehab stays, ensure that all interventions are documented and tracked, and partner with community provider organizations.

Responsibilities

  • Partner with the Wellness Coordinator and the resident services team to provide comprehensive case management services to residents.

  • Provide regular preventative outreach to all residents to check in on their needs and overall health and develop trusting relationships with residents and their families. 

  • Conduct wellness assessments of residents to assess risk and determine needs.  Actively follow up on all identified needs including finding resources, making referrals and ensuring residents are actively engaged in services.

  • Active follow up on all hospitalizations, rehab stays, emergency room visits.  Work with families, hospitals, rehabs, HSL Home Care and/or VNA, ASAP’s and other providers to ensure safe discharges and ongoing services. 

  • Follow up regularly with at risk residents to support adherence to health and wellness related activities, medication and treatment plans.  

  • Conduct and/or coordinate group and individual education sessions on health and wellness, including medication management. 

  • Assess and keep track of the needs of residents with special needs, such as dementia and mental health, and make appropriate referrals.

  • Utilize collected data to identify, plan, schedule and implement focused programs, such as falls prevention.

  • Support and educate housing staff members about common medical conditions and how to identify and communicate status changes.

  • Participate in resident services team meetings, provider meetings and individual family meetings.

  • Assist residents and family members with transition to other levels of care when needed.

  • Assist with specific resident needs such as taking vital signs, educating and assisting with Health Care Proxy and File of Life forms, arranging clinics for vaccines and arranging other health focused clinics, supporting residents in preparing for planned surgeries/medical tests.

  • 3 years experience in aging services required, home health experience and dementia care a plus.

  • Knowledge and experience of property claims contracts.

What sets you apart:

  • Previous experience managing TPA estimating, QA, Audits.
  • Strong analytical skills with demonstrated ability to perform root cause analysis and provide useful insights.
  • 4+ recent years writing Dwelling estimates in Xactimate and XactAnalysis.
  • 4+ recent years of working property claims contract knowledge.
  • Experience working with both internal and external partners/suppliers.
  • Willingness to travel at least one week per month.
  • Currently reside or work within any of the following regions: San Antonio, TX, Austin, TX, Dallas/Ft Worth, TX, or Houston, TX.

Physical Demand Requirements:

  • Ability to lift a minimum of 35 pounds to include lifting a ladder in and out of the trunk of a car.
  • Ability to climb ladders and traverse roofs, this includes the ability to work at heights while inspecting roofs and attics.
  • Ability to crouch and stoop to inspect confined attic spaces and go beneath homes into crawl spaces.
  • Meet all USAA safe driving requirements including verification of driving record through MVR & possession of valid driver’s license.

Compensation range: $77,120-$147,390.
