- Participate in interdisciplinary group (IDG) meetings to support coordinated hospice care.
Additional Responsibilities
- Provide in-service education for nursing and organizational personnel as needed.
- Participate in peer consultation and QAPI activities.
- Supervise Physical Therapy Assistants (PTAs) according to policy and state regulations.
- When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
Qualifications
- Bachelor’s or master’s degree in physical therapy from an accredited program.

Established since the 1940's, physician-led private practice with state-of-the-art technology and innovative diagnostic procedures.
FL has no state income tax.High-volume Urgent Care department supporting patients with walk-ins, after hours, and weekend care.
Urgent Care and Xpress Care departments are supported with a regional hospital with an on-call around the clock emergency services and specialty care.Urgent care department hours are 8am 6pm / 7 days a week.
NO Nights and NO Call.
15 shifts per month.
Must be willing to work 2 weekends p/month.
Procedures include lacerations, I&D, and foreign body removals.
On-site CT, Ultrasound, Digital X-rays, and Full Laboratory Testing.
Work with currently 10 providers and dedicated experienced RNs and LPNs.
Extensive primary care and multi-specialty base for timely referrals.
