Job Description 

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. Qualifications

Job Summary:

·        Demonstrates knowledge of care for required age-specific populations necessary to carry out safe and effective performance of duties.

·        Participate in hospital functions and events that support and health and engagement.  May be required to vary shifts to meet the needs of patients and programs.

·        Travel between various locations within the community to perform duties.

Qualifications

JOB RESPONSIBILITIES

Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.

Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.

Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.

Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.



Qualifications

Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Required

Bachelor's degree in nursing, master's degree in PT, OT, SLP preferred

JOB SUMMARY

The Assistant Nursing manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. Hartford Hospital is seeking a Physician Assistant or Nurse Practitioner to join their energetic hospitalist team. The Hospitalist program is large, vibrant, and growing! The successful Physician Assistant or Nurse Practitioner APRN will provide full-spectrum inpatient care, including triaging, admitting, rounding, discharging, and addressing changes in clinical status. New and experienced providers are met with a comprehensive orientation provided by our exceptional Physician and Advanced Practice Hospitalists

Nurse Practitioner and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Advanced Practitioners perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Advanced Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Advanced Practitioners provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Advanced Practitioner Role Scope  

  • The APP will be dividing their time between SNF setting and infusion/outpatient cardiology clinic.

  • The infusion clinic involves assessment, follow up, instruction and collaboration with the community cardiologist for the necessity of intravenous diuresis, thereby reducing the need for hospitalization, improving QoL, adhering and implementing GDMT.

  • The APP will not only be responsible for instituting a new infusion clinic needed across the central region outside of the current one established at HOCC, but also coverage of cardiac testing.

  • The APP will play a vital role in filling the gaps and needs of our heart failure population, educating our patients and families lifestyle modifications, GDMT and providing an additional resource to the affiliated cardiology practices. 

Shift: Full Time – The hours split TBD by the needs of patient population.

The APP is primarily in SNF setting
