Job Responsibilities:
- Participates in the development of patient physical therapy plans of care.
- Makes visits to the Patients for assessments and evaluations and communicates with the physician before start of care as needed.
- Implements the plan of care.
- Documents the care and submits paperwork in an accurate and timely manner to update and maintain the Patient medical records.
- Provides education and instruction to team members, Patients, and their families.
- May participate in agency quality assessment performance improvement (QAPI) programs.
- Ensures the growth & profitability of the company through the responsible use of company resources and educating the community to our services.
- Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the Patient and family, exchange information & problem solve & receive staff support and education.
- Applies the policies and procedures of the agency and the rules and regulations of federal and state regulatory agencies and other certified agencies in providing care.
Required Qualifications:
- Bachelor's or Masterβs Degree in Physical Therapy.
- Current state license to practice as a Physical Therapy Assistant.
- Current BLS certification.
- Minimum of 2 years of experience as a Physical Therapy Assistant.
- Must have Home care Home base Experience.
- Floating between departments and or local locations within 20 miles is required.

Gestational Carrier Requirements
To ensure a safe and successful journey for both carrier and baby, applicants must meet the following criteria:
- Between 21β39 years old
- U.S. citizen living in a surrogate-friendly state
- Have delivered at least one child and are currently parenting
- No more than two (2) C-sections
- Not receiving government or public assistance (including Medicaid, WIC, SNAP/Food Stamps, or state-funded health insurance)
- No history of pregnancy complications (including gestational diabetes, pre-eclampsia, or pre-term labor)
- No history of schizophrenia, bipolar disorder, borderline personality disorder, or major depressive disorder
- Not currently taking medications unsafe for pregnancy (including certain mental health medications)
- Able to travel for medical screening, monitoring, and embryo transfer appointments
- Live a healthy lifestyle free of illicit or recreational drug use
- Have a stable home environment and strong support system

Compensation & Benefits
We recognize the commitment, time, and care involved in being a gestational carrier. Compensation includes:
Base Compensation
- $60,000 β $70,000
(First-time carrier to experienced carrier)
Additional Benefits
- $250 monthly expense allowance
- $500 maternity clothing allowance
- $10,500 for multiple birth
- Up to $10,000 for loss of organs (per contract terms)
- Lost wages (carrier and spouse, if applicable β based on employment verification)
- Travel reimbursementΒ
- $200 per week for housekeeping (if medically necessary)
- $20 per hour for childcare (if medically necessary)
All medical expenses, legal representation, and services related to the surrogacy journey are covered.
