You will also co-lead the hospital's Trauma Survivorship Program, with a specialized focus on TBI.

This role requires a compassionate, organized, and mission-driven individual who can provide psychosocial support, coordinate multidisciplinary care, and communicate effectively with families and providers.

Responsibilities: As a Consulting Aftercare Coordinator (TBI) Patient Navigator, your responsibilities will include: Serving as the primary liaison for families throughout the TBI recovery process Providing emotional support, education, and resource navigation tailored to individual patient needs Facilitating care transitions between inpatient, outpatient, and community-based services Supporting school reintegration and long-term adaptation to daily life post-TBI Designing and implementing program components in collaboration with trauma leadership Developing and managing project plans, charters, budgets, and status reports Assisting in creating educational curricula for staff and families Participating in trauma-focused process improvement initiatives Leading or contributing to data collection and outcome reporting activities Qualifications: The ideal candidate for the Consulting Aftercare Coordinator (TBI) Patient Navigator role will possess: A license as a clinician such as LCSW, LMFT, RN, PA, or NP 5+ years of experience in pediatric settings, trauma or TBI care, psychosocial support, or behavioral health Experience in supporting families emotionally and coordinating multidisciplinary care Strong organizational skills with comfort in project management and program development Clear communication skills with the ability to educate families, staff, and external partners Flexibility and willingness to support events and improvement initiatives Experience with pediatric care and care of children/families who have had a traumatic brain injury is highly desirable Experience in a multidisciplinary environment is a plus Ability to work occasional evenings and weekends, as needed.
