Pride Health is seeking an experienced Clinical Manager (RN) to join our client, Home Health facility, in Silverdale, WA (98383). This is a 13-week Contract role focused on oversight of patient care, OASIS accuracy, care coordination, and clinical compliance within a high-volume Home Health setting.


Position Overview

The Clinical Manager ensures the client's Plan of Care is accurate, individualized, and consistently updated based on the patient’s condition. This role requires strong Home Health leadership experience, advanced knowledge of HCHB (HomeCare HomeBase), and the ability to manage clinical staff, orders, documentation, and weekly case conferences.


Key Responsibilities

Clinical Oversight

  • Ensure Plan of Care is implemented and updated per patient needs or clinical changes.
  • Review and ensure accurate evaluation packets, OASIS, and 485 forms.
  • Process OASIS assessments, verify SOC dates, and correct documentation discrepancies.
  • Follow up on missing documentation and ensure completion by Licensed Professionals.
  • Manage referral intake and ensure timely scheduling of assessment visits.
  • Lead weekly Case Conferences and guide recertification/discharge decisions.
  • Review and approve Case Conference coordination notes within 48 hours.
  • Communicate with physicians to obtain, confirm, or update clinical orders.

Order Management

  • Review, approve, or decline orders per protocol.
  • Ensure modifications, corrections, and medication updates are completed accurately.
  • Track unsigned orders and follow up as needed.
  • Ensure frequency-related orders reflect correct scheduling.

Quality & Patient Safety Monitoring

  • Review vital-sign alerts and wound-score deviations; notify physicians as appropriate.
  • Enter non-admit information in HCHB and ensure Branch Director approval.
  • May perform patient visits or be included in on-call rotations.

Administrative Duties

  • Verify benefits and obtain authorization for non-Medicare patients.
  • Follow up on billing claim audit deficiencies within 24 hours.
  • Run and submit missed-visit notifications to physicians weekly.
  • Serve as backup for PSC functions (rescheduling, lab report review when allowed).


Required Skills & Systems

Qualifications

  • Education: Graduate of an approved nursing school; Active RN license required

  • Minimum 1 year Home Health experience
  • Clinical Manager experience with HCHB is required
  • Preferred: Supervisory experience
  • Must possess valid driver’s license, auto insurance, and ability to drive in all weather conditions

Local Pay range: $55-$58/hour

Job Summary:

The Dental Hygienist will provide comprehensive preventive and therapeutic dental care at the Naval

Hospital Bremerton Dental Department. This position focuses on patient education, oral health

maintenance, and disease prevention, supporting the Navy’s mission to ensure the oral readiness and

overall health of service members and their families. The Dental Hygienist will work collaboratively with

dental officers and clinical staff to deliver high-quality care in compliance with Navy, federal, and

professional dental standards.

Duties/Responsibilities:

• Examine patients’ oral cavities—including mouth, throat, and pharynx—and record conditions of

teeth, gums, and surrounding tissues.

• Identify and refer patients with abnormalities such as cavities, defective fillings, suspicious

lesions, or periodontal disease to the supervising dentist for evaluation and treatment.

• Apply desensitizing and topical agents to aid in the prevention or treatment of caries, gingivitis,

and oral ulcerations.

• Provide oral hygiene instruction to individual patients and groups, demonstrating proper

brushing and flossing techniques, explaining the causes of tooth decay, and discussing the role

of nutrition and general health in oral care.

• Take impressions and fabricate sports/safety mouth guards and soft night guards for use in vital

bleaching or protection.

• Polish restorations and apply pit and fissure sealants as indicated.

• Assist chairside during dental procedures when directed by the Officer-in-Charge of the dental

clinic.

• Record treatment data, maintain patient documentation, and ensure instruments and

equipment are cleaned, sharpened, sterilized, and in proper working order.

• Utilize Dentrix to accurately enter patient-specific workload data, readiness status, and to

schedule patient appointments in accordance with Navy protocols.

The job duties are flexible and subject to change depending upon operational needs.

Job Summary

PetSmart’s Store Leader has leadership oversight of the safety of people and pets, pet parent experience, operational execution, financial outcomes, and all human resource functions within all areas of the store.   • Cascades information to associates and adjusts team priorities to meet goals and commitments.

  • Address and administer associate complaints and grievances.

Overall Store Experience: 

  • Leads the associate and pet parent experience at all levels and ensures pets in our care are safe and healthy.  
  • Accountable for the pet parent experience and outcomes conducted over the telephone, in person or online, and responding to general and escalated pet parent concerns. 
  • Builds a store environment that is focused on consistently delivering an exceptional, positive in-store pet parent and associate experience. 
  • Evaluates operations and seeks opportunities to continuously improve processes and services. 
  • Collaborates and partners with the Store Leadership team to ensure efficient and effective outcomes of in-store and community events including partnering with local adoption groups. 
  • Maintains store standards and leads a culture of empowerment by ensuring compliance with our policies and procedures (P&Ps) and code of ethics for themselves and their team. 
  • Responsible for conducting monthly Core and Services Walks to validate proper execution of policies and procedures.

  • Communicates business and financial objectives to other leaders within the store.  
  • Responsible for reviewing in-store cameras to assess safety practices, associate behaviors and assess risk. Follow up with associates and leaders with coaching and accountability as appropriate. 
  • Responsible for taking immediate action and reports when a sick/injured pet is identified in the store; transport to the vet as needed. 
  • Shared responsibility with store opening and closing procedures to ensure we uphold our brand promise. 

Business Management:  

  • Deliver corporate strategy and profitability goals by executing the direction set leadership.  
  • Responsible for analyzing, root cause and responding to a variety of reporting in all areas of the store. Communicates business results to leaders and associates; creating improvement plans where needed 
  • Identifies and drives opportunities to grow the business, recognizes business successes and holds associates accountable in achieving store P&L expectations such as sales and shrink targets. 
  • Manages expenses such as labor and supplies and any other relevant store metrics. 
  • Leverages home office partners and resources to submit and follow up on store related work orders and purchase card allocation. 
  • Creates and posts schedules for the entire store.  
  • Accountable for processing payroll weekly and managing labor within a budget. 
  • Executes daily, weekly, and monthly reporting and paperwork as required by the Company. 
  • Accountable to run store related errands to support store needs. 

Qualifications

  • 4-6 years of retail leadership or experience in a customer-focused environment. 
  • Full time availability required, flexibility in schedule, able to work evenings, weekends, and holidays as needed.  
  • Proficiency in computer applications. 
  • Strong written and verbal communication skills. 
  • Ability to react under pressure and maintain composure.   
  • Strong organizational skills and attention to detail.  

People leadership:

  • Typically oversees the Experience Leader and Process Leader, up to six lead associates and ten to seventy non-leader associates. Carries out supervisory responsibilities in accordance with PetSmart’s policies and procedures.  

Physical demands and work environment

  • Associate required to stand, walk, climb a ladder, and use hands to handle, feel, hold, write, etc. The associate frequently is required to reach with hands and arms: stoop, kneel, crouch, talk and hear. Essential Functions & Responsibilities

  • The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
  • Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
  • In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
  • The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
  • Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
  • Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
  • Physician must be able to supervise collaborating nurse practitioners and physicians assistants


Compensation
