In collaboration and consultation with physicians and other health care professionals, the nurse practitioner provides evidence-based, quality, compassionate and holistic care to in-patients and their families.

POSITION SUMMARY/RESPONSIBILITIES
The RN Service Coordinator-RN (RN-SC) contributes to the Long Term Services and Supports (LTSS) service coordination process by performing activities within the scope of licensure; provides supervision and direction to staff participating in Member's cases following applicable state law and contract; develops, monitors, evaluates, and revises the Members' care plans to meet Member's needs, to optimize health care across the care continuum to enhance the Member's well-being, independence, integration in the community, and potential for productivity. The RN-SC conducts a holistic assessment of the Member's dynamics, needs, and preferences while providing education and health-related information to the Member, the Member's Legal Authorized Representative (LAR), and the Member's Support Network. Responsible for the coordination of STAR+PLUS Members' covered and non-capitated services, including acute and LTSS, while meeting the Member's physical, behavioral, functional, and psychosocial needs. Complies with policies, procedures, and protocols for establishing and maintaining good working relationships with co-workers, employees, patients, and guests.
EDUCATION/EXPERIENCE
Graduation from an accredited school of professional nursing or social work is required. BSN is preferred. Four (4) recent years of clinical experience preferred, which may include service coordination, case management, quality management, or managed care experience. Working knowledge of HMO standards, LTSS, and NCQA standards is preferred. Knowledge of Medicare and Medicaid HMO experience is preferred. Experience in meeting the needs of vulnerable populations who have chronic or complex conditions is required. Bilingual, English/Spanish, is preferred.
LICENSURE/ CERTIFICATION
A current, unrestricted license to practice professional nursing issued by the State of Texas is required. RUG Certification is required and must be obtained within 30 days of employment for all RN candidates.
Provides skilled direct patient care (dialysis) and monitoring of renal dialysis patients.
Operates all renal dialysis equipment and recognizes changes in the renal patient's condition while on dialysis.
Administers medications and access hemodialysis catheters for the patient care techs as assigned.
Three years recent experience in general patient care and successful completion of 3 months probation period is required.
Verification of course completion in accordance with all American Heart Association Basic Cardiac Life Support and Health Care Provider guidelines is required.

EDUCATION
POSITION SUMMARY/RESPONSIBILITIES Serves as member of health care team that is responsible for administering anesthesia and monitoring patients during surgeries and other medical procedures.
Serves as a subject matter expert in critical care and airway management.
Functions in a variety of inpatient and outpatient settings including ambulatory surgery centers (ASC).
EDUCATION A Registered Professional Nurse who is prepared for advanced nursing practice by virtue of knowledge and skills obtained through an accredited post basic or advanced educational program of study acceptable to the Board of Nurse Examiners.
Graduation from an accredited school of nurse anesthesia.
Must be a participating Medicare provider or eligible to obtain a Medicare provider number.
LICENSURE/CERTIFICATION Must be currently licensed as a Certified Registered Nurse Anesthetist in Texas and registered by the State to practice as an Advanced Nurse Practitioner.
Must maintain current AHA ACLS or higher in accordance with Medical-Dental staff bylaws.
Certification by a national nursing body is recommended.

EDUCATION/EXPERIENCE
MSNis required plus specialty national certification and Nursing Professional Development Certification or Healthcare Simulation Educator Certification is required or PhD plus specialty national certification is required. Minimum of five years of full-time nursing experience with at least three years in related nursing specialty is required. Demonstration of experience in curriculum development and implementation of adult learning principles is required.
Performs the nursing process in a safe therapeutic manner in a designated clinic setting.
Assumes the responsibilities of leadership in varied clinical settings to ensure efficient and appropriate administration of care.
Supports and maintains the University Health as well as the clinic's departmental policies and standards.
BSN is highly preferred.

POSITION SUMMARY/RESPONSIBILITIES Assists Community First Health Plan (CFHP) members regain optimum health or improved functional capacity by ensuring that members have access to all of the health care services they need in the most efficient and effective manner possible.
Responsibilities include but are not limited to overseeing the allocation of resources, cost and quality of health care for members; coordinating care between the primary care physician, community resources, family and member; coordinating care across the health care continuum while monitoring and managing benefit utilization; and, collaborating with multi-disciplinary health care team members in identifying the educational and discharge needs of members.
EDUCATION/EXPERIENCE Registered Nurse (RN) is required.
Bachelor of Science in Nursing (BSN) or Master's degree is preferred.
Minimum three (3) years nursing, acute care, quality management or managed care experience is required.
Basic knowledge of Medicaid, Medicare, community resources and alternate funding programs is desired.
Knowledge of InterQual screening criteria as well as DRG, ICD and CPT coding is preferred.
LICENSURE/CERTIFICATION Current licensure as a Registered Nurse with the Texas State Board of Nurse Examiners is required.
Current certification from an appropriate professional agency, such as Case Management Society, is preferred.
