Position Summary

The Physician Assistant (PA) or Nurse Practitioner (NP) will work as a key member of our primary care team, delivering high-quality, patient-centered care. This role involves direct patient evaluation, diagnosis, treatment, and follow-up across a variety of acute and chronic conditions, with opportunities to contribute to practice development, patient education, and community health initiatives.

Key Responsibilities

  • Conduct comprehensive health assessments, physical exams, and preventive screenings.
  • Diagnose and manage acute and chronic conditions in patients of all ages.- Order, interpret, and act on diagnostic tests and lab results.
  • Prescribe medications and manage pharmacologic and non-pharmacologic treatments.
  • Provide counseling and education to patients and families on health maintenance, disease prevention, and lifestyle modification.
  • Collaborate closely with supervising physician(s), nursing staff, and administrative staff to ensure seamless patient care.
  • Document encounters and care plans accurately in the electronic medical record (EMR).
  • Participate in quality improvement projects and contribute to the development of practice protocols.
  • Maintain compliance with all federal, state, and local regulatory standards, as well as ethical and professional practice guidelines.
  • Support patient access initiatives, including same-day visits, telehealth services, and chronic care management.

Qualifications Education & Licensure: Graduate of an accredited Physician Assistant or Nurse Practitioner program. Current licensure in the state of Vermont. National certification (NCCPA, AANP, or ANCC). DEA registration (or eligibility).

Experience: Preferred: 1?2 years of clinical experience in family medicine, internal medicine, or urgent care. New graduates with strong clinical training and demonstrated commitment to primary care are encouraged to apply.

Skills & Competencies: Strong diagnostic and clinical decision-making skills. Excellent interpersonal and communication abilities. Proficiency with EMR systems and comfort with telemedicine platforms. Ability to work independently while maintaining a collaborative team approach. Commitment to patient-centered, culturally sensitive care.

Pay: $50.00 - $75.00 per hour
