Responsibilities
The Registered Nurse (RN) Case Manager works within an interdisciplinary team to facilitate the patient plan of care throughout the continuum of care by ensuring appropriate utilization management, care coordination, resource utilization, and clinical documentation. The RN Case Manager will function within the Mayo Clinical Nursing Professional Practice Model, which includes accountability for assessing, planning, implementing, evaluating, and communicating the patient care plan progression. The RN Case Manager utilizes the principles of mutual respect, patient/family advocacy and provides leadership within the team of internal partners and outside agencies to facilitate best practices that achieve quality clinical, financial, and patient satisfaction outcomes. The RN Case Manager provides leadership through education on case management/utilization management concepts, committee work, research, and community involvement. The RN Case Manager bridges communication between providers, patients and families, members of the interdisciplinary team, and outside agencies to assure high-quality care that meets the patient's needs and is delivered in a cost-effective and timely manner. The ANA Nursing: Scope and Standards of Practice and Code of Ethics provide a basis for the practice of the RN. Qualifications
Graduate of an accredited, or those in the candidacy process, baccalaureate nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). One year of RN experience required.
Additional Qualifications:
Three years recent acute medical/surgical hospital nursing experience preferred. Must have in-depth understanding of anatomy, physiology medical terminology and disease processes. Knowledge of CPT coding, resource management systems, electronic medical record and personal computers beneficial. Demonstrated leadership, independent problem solving, conflict resolution, critical thinking skills, and team building skills required. Must be self-motivated and able to work as part of a team with minimal supervision. Must possess the ability to work well within a creative and challenging work environment. Experience in utilization review, ability to navigate medical records, value based purchasing metrics, or pre-certification helpful. Additional nursing experience in specialty area beneficial. Mayo Clinic's Division of General Internal Medicine in Rochester is one of the largest academic general internal medicine divisions in the world - serving as the gateway in evaluating and caring for adult patients that may have unexplained symptoms and/or complex medical conditions. Consultative Medicine Clinic (a section of GIM) provides consultative medical care to patients that reside outside of Olmsted county in Minnesota. Consultative Medicine sees new patients to Mayo Clinic for an initial comprehensive evaluation. The LPN role in Consultative Medicine includes, but is not limited to, patient visits (face to face and via telephone) review of patient health history, medication review, immunization administration, focused nutrition assessments, and blood pressure visits. Telehealth interactions may include collecting pertinent information prior to their arrival to the clinic, researching prescription discrepancies, and/or facilitating prior authorization denials. The LPN collaborates daily with physicians, nursing colleagues, and support staff.

Primary Care patient services include health care for existing medical needs, preventative care, acute care, and management of chronic health conditions. Care is provided through in-person visits, phone, video and web-based care. LPNs in all primary care practices are valued members of the interprofessional care team, collaborating with physicians, advanced practitioners, non-licensed allied health professionals, registered nurses, and student learners. Primary Care locations offer a fast-paced, team-oriented environment that allows LPNs to work closely with an interdisciplinary team while building meaningful, long-term relationships with patients
Adults 18+ with focus on geriatric and palliative needs, including chronic condition management and nurse-led procedures. Community Internal Medicine, Palliative and Geriatrics (GIM-GP) cares for patients age 18 and older. LPNs in Community Internal Medicine play a vital role in delivering patient-centered care for adult/geriatric patients. The LPN is responsible for after-visit care for provider appointments and managing nurse visits through the LPN calendar including; administering immunizations and medications, performing ear lavage, post-void residuals, intermittent and indwelling catheter care, wound care and removing sutures or staples, reinforcing patient education. Responsibilities
The Ambulatory Float Licensed Practical Nurse (LPN) works in variety of dynamic specialty clinics and primary care including, but not limited to, the following locations: Baldwin, Gonda, Mayo Clinic, NE Clinic, NW Clinic, SE Clinic, and Kasson. Ambulatory floats provide direct patient care to support the unique needs of the clinical practice. The Ambulatory Float Pool supports 16 different LPN practices serving patients from pediatrics through adulthood. Hours are Monday through Friday, with varied start times from 7-8 am. The majority of the schedule is 7:45-4:45 based on unit need.

The Pulmonary Medicine adult ambulatory practice meets the adult patient’s needs with acute and chronic pulmonary disease. The unit is staffed with consultants, fellows, NP/PA, registered nurses, procedure assistants, respiratory therapists and Internal Medicine residents . Services include but not limited to medical management, education, same-day interventional procedures and managing in basket messages.
The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. Qualifications
Graduate of a nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). One year of RN experience required.
All entry-level associate degree registered nurses with a RN start date of April 1, 2020 and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within five years from the last day of the month of the RN start date.
All entry-level associate degree registered nurses with a RN start date prior to April 1, 2020 must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program as stipulated by the degree requirement program in place at time of hire.
Additional Qualifications:
1 year professional RN experience preferred
Minimum of two years previous RN experience preferred. Advanced Cardiac Life Support (ACLS) and EKG experience preferred. Experience with moderate sedation is beneficial. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Additional specialty certification/training as required by the work area.
Internal applicants are required to attach their three most recent performance appraisals.
License and Certification (Must obtain prior to start date):
Current RN license by applicable state requirements.
BLS (Basic Life Support) must be valid for 90 days post start date; acceptable certifications include: American Heart Association (AHA)/ Heartcode® BLS, Military Training Network, Red Cross (CPR/AED for Professional Rescuers and Health Care Providers).
Additional state licensure(s) and/or specialty certification/training as required by the work area.
