What to expect:
Provides direct clinical care, participates as a team member interdisciplinary patient care programs, keeps current with the latest physical therapy techniques that are appropriate for the patients of Johns Hopkins Home Health Services. Provides in-services to health care personnel within Johns Hopkins Care at Home as well as to outside groups regarding current acceptable physical therapy techniques.
Shift: Exempt, Dayshift Full-time 40 hours per week
Location: Based in West Baltimore City & County
Requirements/Qualifications:
- B.S. - M.S. degree in Physical Therapy required; DPT preferred
- PT license in the state of Maryland
- Active CPR certification in Adult and Pediatric CPR
- One year experience in clinical care, acute/subacute/home health preferred. Home care experience preferred.
- Valid Maryland Driver's License and current auto insurance, working vehicle; agree to comply with Maryland law for mandatory seat belt use.
Salary Range: Minimum $37.02/per hour - Maximum $61.11 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.

The Neonatal Nurse Practitioner or Neonatal Physician Assistant works collaboratively within a multidisciplinary health team and is responsible for providing comprehensive and age appropriate care to both well and/or ill patients. The NNP or PA educates patients' families and/or intervenes in acute or chronic illness. The NNP or PA practices within their scope as defined by their education, specialty certification and the guidelines of the unit, department and institution. The NNP or PA is granted affiliate staff privileges by the Johns Hopkins Hospital Medical Board; these privileges reviewed during re-credentialing.

Schedule: Rotating, Days and Nights, Weekends and Holidays
Responsibilities:
- Conduct thorough assessments of neonatal patients.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
- Monitor and manage the ongoing care of patients, adjusting treatment plans as needed.
- Educate and support patients and their families in understanding and managing complex medical conditions.
- Serve as a resource and mentor to other healthcare team members.
- Participate in case conferences and collaborate with specialists to ensure coordinated and comprehensive care.
Qualifications:
- Master's or Doctorate degree in nursing from an accredited NNP program, or Masters in Physician Assistant Studies from an accredited PA program
- Registered Nurse (RN) license and Nurse Practitioner (NP) certification, or PA
- Maryland or Compact RN and CRNP, or MD PA
- Current BLS CPR certification
- Minimum 3 years of acute care RN NICU experience required; Level III or IV NICU experience preferred. PA: Level III or IV NICU experience or completion of a Neonatal Residency or Fellowship.
Salary Range: Minimum 55.81 per hour - Maximum 86.51 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

• Successfully complete a criminal background check, TB test, and pre-employment medical examination.
• Experience with substance abuse disorders, preferably with women.
• Ability to work with clients who have experienced trauma.
Duties and Responsibilities:
• Provide Individual and Family Therapy.
• Prepare all required paperwork during the agreement period.
• Outline and coordinate client treatment plans.
• Participate in Treatment Team Planning and Meetings.
• Ensure timely submission of documentation regarding client and/or family contacts.
• Complete required training and maintain contractor HR file with current documentation.
• Respond to emails, calls, and messages within 1 business day.
• Provide services at client-preferred locations.
• Use FSW for interpreting sessions, avoiding the use of client/family members or phone apps.
• Accommodate evening sessions.
• Ensure clinically appropriate client visit frequencies (weekly, bi-weekly, bi-monthly, or monthly).
• Encourage and document yearly Psychiatric Evaluations for clients.
• Discharge clients following outreach attempts and request management approval after 30 days of inactivity.
• Comply with Key Performance Indicators including timely clinical documentation, productivity, meeting attendance, community outreach hours, research and development/social justice initiatives, committee participation, and timely HR documentation.
• Attend mandatory agency trainings and individual supervision as required.
• Participate in semi-annual Performance Evaluations.
