Nurse Practitioners & Physician Assistants - Job Description:

  • Conduct in-person home visits to manage acute illnesses and injuries.
  • Provide treatment and clinical management consistent with evidence-based guidelines.
  • Work collaboratively with our multi-disciplinary team of field providers and care coordinators to efficiently manage complex care needs in the home.
  • Assesses patients for risk factors associated with the care of the acute and complex chronically ill patient including: medication compliance and side effects, limited mobility, impaired nutrition, fall safety, and social isolation.
  • Maintain a safe, secure, equitable and healthy work environment by following safety procedures, quality standards and comply with regulations
  • Maintain patient confidentiality
  • Complete patient records in the Electronic Health Record in a timely manner that meets acute care compliance standards and captures patient acuity.
  • Respond to on-demand and scheduled calls and focus on quality and efficiency metrics to improve patient experience.
  • Stay current on in-home acute care literature, technology, and regulatory issues.

Nurse Practitioners & Physician Assistants - Education and Experience:

  • Master?s Degree in PA or NP Studies from an accredited university.
  • Valid PA or NP Certification and license to practice in New York.
  • Able to meet credentialing requirements, including DEA certificate, and BLS certification.
  • Experience in providing care in an emergency department or hospitalist medicine practice.
  • Additional experience in geriatric medicine, hospital at home, or post-acute care a plus.

Physical Requirements:

  • Daily use of PPE and electronic documentation.
  • Frequent, prolonged standing, walking, and patient/equipment transfers.

Job Description

Summit Health is seeking a Physician Assistant to join our established Plastic Surgery department at our Purchase, NY location. This Physician Assistant (PA) will function as a patient care provider within the outpatient office, hospital, and operating room and will have responsibilities including: Helping the provider to see patients in the office, seeing patients independently in the office, assisting in the operating room and assisting in hospital rounding, when applicable.

  • Perform patient consultations and develop customized treatment plans
  • Administer injectable treatments (neurotoxins, dermal fillers, collagen stimulators, PRF, etc.)
  • Perform treatments using advanced energy-based devices and lasers (training provided on all our devices)
  • Educate patients on skincare regimens and treatment protocols
  • Collaborate with the medical director and management team to maintain the highest standard of care
  • Participate in marketing events, staff education, and community outreach as a brand ambassador
  • Must be able to commute to Avon, CT periodically for full staff trainings (meetings and some education can be done virtually)
  • Be a team player! Nobody is above vacuuming or taking the trash out

Qualifications

  • Current Connecticut licensure as a PA-C or APRN - no RNs/LPNs please
  • Current DEA & CT controlled substance license
  • 3+ years of aesthetic medicine or relevant medical experience (required)
  • Skilled in a broad range of injectables; laser/device experience preferred (training provided)
  • Strong communication and leadership skills
  • Comfort working independently in a fast-paced, high-end clinical environment
  • Bonus: background in wellness, skincare, or integrative health

Compensation

  • $70 ? $85 per hour (based on experience)
  • Commission on services and products
