POSITION PURPOSE 

The Case Manager coordinates care across an episode and/or the continuum for clients with complex problems and diverse needs.  The Case Manager’s focus is to maintain patients at an optimal level of health and to support self-care.  Case Managers collaborate with physicians, social services, nurses, and community agencies to define care options and resources, to plan cost effective quality care and to achieve optimal outcomes. 

 

Specific responsibilities include case screening, insurance approval, assurance of timely services, and facilitation of discharge with transition to the appropriate services.  Patient outcomes are achieved through effective application of care plans, managed care concepts, appropriateness criteria, resource management, knowledge of community resources, and collaboration with other clinical disciplines.  Works proactively to coordinate the services of physicians, nurses, and other disciplines to effectively prepare patients for discharge.  The Case Manager facilitates program development, efficient care delivery processes and quality improvement including tracking of resource utilization and outcome measures.  The Case Manager is accountable for improving service using cost and quality outcome data, current clinical practices and related research, regulatory requirements and comparative benchmark opportunities.   

 

Customers include patients, families/visitors, physicians, physician assistants, nurse practitioners, case managers, representatives of third-party payers, representatives of referring agencies, interdepartmental and intradepartmental staff, vendors, and volunteers.

  • Attends meetings of the Utilization Review Committee and submits reports as required.  Participates in the development of a written plan that describes the Utilization Review Program. 
  • Applies quality improvement methods and techniques to improve case management processes to maximize cost and quality benefits for MercyOne Medical Center.

JOB SUMMARY

The home health registered nurse uses the nursing process (assesses, plans,

  1.    Education:  Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program.

  3.    Experience:  One year RN experience and a total of 2 or more years clinical experience is required.  Supplemental experience may include experience as LPN, CNA, military medic, EMT or related experience.  Home health experience preferred. Less than 1 year RN experience requires 1 year of clinical experience as LPN (Internal use only).  Therapy Assistants (PTA, OTA) with 1 year of Home Health experience and at least 6 months RN experience (internal use only).

  4.    Essential Technical / Motor Skills:  Hand/eye coordination in order to give injections, use computer, etc.  Must be able to communicate and be literate in the English language.  Able to manipulate patient care equipment, to properly transfer and guard patients.

  5.    Interpersonal Skills:  Ability to develop positive interaction with patients, patients’ families, physicians and staff in order to effectively care for the patients.

About the Unit:  

The laboratory at Mount Carmel Dublin will provide a wide range of laboratory procedures that will aid clinicians in diagnosing, treating, and managing patients. There will be comprehensive testing in the specialties of hematology, chemistry, coagulation, urinalysis, blood gases, molecular, and blood banking.  Located on the 1st floor will be an Outpatient Lab Service Center that will provide convenient hours for patients to have their blood drawn.  Self-scheduling will be available to allow for little to no wait times.  This laboratory will house all new state of the art instrumentation along with EPIC Beaker laboratory information system.  The laboratory team will interact with other Dublin Hospital health care professionals to coordinate the best patient care experience. 

About the job:  

The Medical Technologist performs laboratory testing. Has knowledge of the clinical significance of results, monitors and understands operation of instrumentation, and reports results accurately and

rapidly.

What you'll do:

  • Performs all clinical testing in assigned department.
  • Records and reports test results accurately and promptly.
  • Performs and interprets quality control testing following established guidelines.
  • Maintains a high level of quality assurance.
  • Practices established safety procedures, including Universal Precautions and proper use of safety equipment.
  • Able to recognize a problem by following prescribed strategies and make necessary corrections.
  • Where no present criteria for decisions are available, will consult with supervisor/manager. In absence of supervisor/manager, will act to determine final decision.
  • Coordinates and evaluates new reagents, procedures and equipment as requested by supervisor/manager.
  • Performs routine scheduled and specialized maintenance of laboratory equipment.

What we're looking for:

  • Education: Baccalaureate or Associate Degree in Medical Technology or related field.
  • Licensure / Certification: Certification from American Society of Clinical Pathologists, equivalent; eligible for certification.
  • If applicable, must maintain active certification.
  • Experience: Preferred, but not required.
  • Effective Communication Skills

Provides perioperative patient care, specifically in the intraoperative phase, in both the scrub and circulating roles according to established guidelines.  Serves the population from neonate, infant, pediatric, adolescent, adult and geriatric.  This position will be full-time exempt and paid a weekly salary to provide on-call coverage weeknights M-F, starting at 1800 each evening and ending at 0700 the next morning.  A commitment to work one 10-hour shift a pay period M-F between the hours of 0600 and 1800 will be in place at the discretion of the facilitator/manager.

ESSENTIAL FUNCTIONS

Administers nursing care to patients; includes assessment, planning, implementing, and evaluating the patient’s care plan in coordination with the physician Scrubs and circulates on cases independently, with minimal direction (OR) Prepares for surgical cases in an efficient manner, anticipating needs of patient, surgeon, anesthesia and other O.R. team members (OR) Provides a safe environment for patient and staff by implementing standards of aseptic technique and by controlling the physical and psychological aspects of that environment. Collects and analyzes data and assesses patient in developing nursing diagnosis. Communicates effectively with the patient, staff, physicians, and families. Maintains knowledge of equipment set-up and function. Initiates and directs room clean up and turnover. Participates in orientation and training of new personnel. Assists in maintenance of physician's preference cards and other O.R. resource material. Utilizes computer in surgical documentation, disposition of specimens and obtaining pertinent information. Participates in departmental Performance Improvement activities as appropriate. Attends department meetings and in-services. MINIMUM QUALIFICATIONS

Possession of a current or compact state license as a Registered Nurse issued/defined by the State of Iowa.  Operating Room experience preferred. Basic Life Support certified within six (6) weeks of hire.  Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers. Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire. Ability to read, write and speak English language. Ability to maintain a high standard of courtesy and cooperation in dealing with co-workers, patients and visitors, and perform job functions satisfactorily despite the stress of a hospital work environment.   • Climbing: while using stairway between locker area and the O.R. and between Inventory Control and the O.R.

PHYSICAL DEMAND REQUIREMENTS:

  • If a patient is unable to function independently, a mechanical/assistive device and/or additional personnel must be utilized.  If an object requires lifting > 50 lbs., additional equipment/personnel must also be utilized.
  • Heavy work: Exerting up to 65 lbs. push/pull (force to move frequently, and lifting up to 50 lbs. occasionally, and lifting up to 40 lbs. frequently, and lifting up to 20 lbs. constantly, to move patient and/or objects.

VISUAL ACUITY REQUIREMENTS:

  • Preparing and reading written and electronic documentation
  • Working with small sutures, needles, etc.
  • Good hand-eye coordination.
  • Peripheral vision.
  • Ability to work with a microscope.
  • Subject to extremes in lighting.

EQUIPMENT/TOOLS: Standard medical and nursing equipment, special equipment or office equipment found in the assigned area. WORKING CONDITIONS:

  • Colleagues in this job classification have been identified as having the likelihood of occupation exposure to blood or other potentially infectious materials and, therefore, are included in the OSHA Exposure Control Plan with all its specifications for preventing contact with the above materials.
  • The worker is subject to inside environmental conditions.
  • The worker is subject to noise from various types of surgical equipment
