You will function as part of a care team who, working collaboratively with a multidisciplinary health team, will be responsible for providing comprehensive patient care. As a successful candidate, you will:

  • Perform history and physical assessments of patients that are accurate, concise, relevant, and appropriate.
  • Document the consultation and new patient note.
  • Perform routine procedures, pertinent to the practice and as granted through the privilege process.
  • During simple follow-up cases, the you may independently manage patients in follow-up including ordering follow-up imaging, providing medical care, and support to the patient including ordering pharmacological agents.
  • Interpret imaging and communicates with attending physician regarding interval patient change.
  • Prescribe and council patients on narcotic use safety.
  • Demonstrate pharmacologic knowledge including indications, contraindications, side effects, interactions, and adverse effects of pharmacologic agents.
  • Review treatment and therapy plans with patients and appropriate family members.
  • Practice clinical care informed by evidence-based literature.
  • Instruct and counsel patients in matters related to health care management including diet, medication and physical and mental health needs and refers to the appropriate specialists when appropriate.

Your qualifications should include:

  • Graduate of an accredited Physician Assistant program with a master?s degree as a Physician Assistant.
  • Graduate of an accredited Nurse Practitioner program with a master?s degree as a Nurse Practitioner

Assist in motivating behavioral changes in patients to improve health outcomes through education, peer support, housing & resource navigation, and the relaying of shared experience.

The Physician Assistant will provide direct patient care to people experiencing homelessness with partner homeless services agencies we work closely with. Meet with patients, perform physicals, discuss symptoms, and educate patients on their condition. Diagnose illnesses, order and interpret laboratory tests and x-rays, prescribe medications, develop and manage treatment plants, and advise patients on preventative care and optimal health practices. Care is delivered directly on the street, encampment, or shelter in the Northern LA County region.

This position requires the use of personal vehicles going out with the teams in the following schedule:

  • Monday -Friday: with the Union Station team which alternates days in cities in the SPA 3 area, such as Altadena, Irwindale, Pomona and El Monte.

**Must be able to perform field phlebotomy and perform minor procedures. Must be able to walk distances and potentially hike to reach encampments as well.
