Duties

Requirements

JOB DESCRIPTION
Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum. Essential Job Duties
• Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management based on clinical judgment, changes in member health or psychosocial wellness and triggers identified in assessments.

Required Qualifications:
- Graduate of an accredited LVN Program
- Current LVN License
- License Vocational Nurse in the State of Texas
- Current BCLS
- Ability to travel throughout the greater Houston area to facilities and patient homes
- Driver's license and access to reliable transportation and current insurance coverage
Preferred Qualifications:
- 1+ years of Home Health experience
- Care Management/ Disease Management
- EPIC experience
- Windows based computer skills
