What you will need:

Required:

  • A current license to practice as a Registered Nurse (RN) in the State of New York
  • Associate’s degree in nursing or Degree of Nursing from an accredited school
  • Minimum one year of experience in a skilled nursing setting as a RN, preferably in a leadership role

Preferred:

  • Managerial/Leadership experience strongly preferred, skilled nursing leadership experience is a plus
  • Bachelor's degree of nursing

Full-Time Hospice Homecare/Case Manager RN (Southwest Albany/Schenectady)


The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing in the Southwest Albany-primarily Delmar, Berne, Altamont, Selkirk, Voorheesville and sometimes Schenectady county areas.   This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

What you will do:

Work as part of our Hospice Homecare Team caring for families residing in the West Albany and Schenectady County region.

Responsibilities:

  • Coordinate with an interdisciplinary team,  to develop a comprehensive plan of care to provide added support to patients and families in their homes. 
  • Work closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

What you will need:

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • 1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and Reliable vehicle
  • The RN must be able to communicate effectively, verbally and written
  • High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
  • Understanding of Hospice principles preferred

Training provided in end of life care.

Pay Range: $79,040 ($38/hr)- $104,374 ($50.18/hr) Pay is based on experience, skills, and education. Full-Time Hospice Homecare/Case Manager RN (Southwest Albany/Schenectady)


The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing in the Southwest Albany-primarily Delmar, Berne, Altamont, Selkirk, Voorheesville and sometimes Schenectady county areas.   This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).

Key Responsibilities:

  • Facilitate orientation, skills validation, and ongoing education for CNAs and nursing staff.
  • Provide hands-on coaching on clinical skills, resident care procedures, and regulatory requirements.
  • Assist with CNA training, competency evaluations, and support for new hires entering the CNA pathway.
  • Partner with leadership and unit managers to identify training needs and reinforce best practices.
  • Maintain accurate documentation of education, competencies, and regulatory compliance.
  • Serve as a clinical resource to ensure adherence to quality, safety, and infection control standards.

Requirements:

  • Valid NYS RN license

  • Associate's Degree in Nursing from an accredited Nursing program

  • 3-5 Years related experience in Long term care facility as staff development coordinator (preferred)

  • Strong communication skills

  • Group and curriculum development skills required.

  • Familiarity with DOH regulations and survey preparation.

  • Must enjoy teaching and be comfortable working with staff at all levels.

Pay Range: $40.00 - $52.81 per hour
