The Case Coordination Department at Hartford Hospital is responsible for the assessment of patient needs for the most appropriate level of care across the continuum. Our staff consist of Registered Nurses experienced in discharge planning and case management and embrace a universal concept of patient advocacy. They are well supported by a team of administrative assistants of various roles and functions enabling a smooth and timely transition for all patients to the next level of care.
Position Summary:
The primary focus of this position is on facilitating patient throughput. As a Case Coordinator at Hartford Hospital, assigned to a select inpatient population, you will assess patients for appropriate level of care; coordinate their care and services across the care continuum; identify risk factors; facilitate involvement of support services; perform utilization management functions; develop and implement transition planning activities; utilize and document aspects of care in our case management specific computer applications; and collaborate with other health care team members on process and quality improvement initiatives. Supervises the program to ensure that a high quality, cost-effective service is delivered by the department and that is in keeping with Hartford HealthCare at Home's strategic plan as well as the Program’s goals and objectives. Introducing this excellent opportunity to join the obstetric team at Hartford Hospital. Our Labor and Delivery unit is a high risk referral center for the area and performs approximately 4,000 deliveries per year. Our Obstetric Triage unit evaluates more than 3,000 patients per year. As the volume and acuity of our patients has grown, we are looking to bolster our inpatient obstetric service with 24/7 APP coverage.
Strong clinical reasoning and excellent communication skills required. This role functions both independently and in collaboration with nurses, residents, and attending physicians. This position will report to the OB/GYN Hospitalist Director.
Advanced Practitioner Role Scope:
Care for patients admitted or being triaged across our Obstetric service line.
Help on Labor and Delivery with admissions, magnesium sulfate checks and labor checks when residents unavailable, during sign-out and periods of higher volume.
Rounding and evaluating our antepartum patients as needed based on volume and resident coverage. Evaluation may include concerns for labor, elevated BP’s, infection or medication orders. Help with biophysical profiles (BPP’s) for non-reactive NST’s and limited obstetric ultrasound
Typical postpartum care includes rounding, discharge planning, coordination of inpatient consults, responding to elevated blood pressures, postpartum or postoperative concerns, medication reconciliation and placement of Nexplanon.
If needed in triage you will evaluate pregnant patients for pre-term/term labor, rupture of membranes, bleeding, high blood pressure, pre-eclampsia, abdominal pain and a variety of other complaints. There is always an attending assigned to cover our service serving as backup to the residents and APP’s.

The Registered Nurse utilizes psychiatric/chemical dependency nursing skills as a direct client care provider and is an integral member of the multidisciplinary team. The RN works under the direction of the Nurse Manager and/or the RN Designee and is responsible for the delivery of direct client care to clients individually, in groups and in the milieu. The RN is a role model for other staff members and is a leader in teaching other staff, in program development, in guiding group decision making, in developing and implementing treatment plans and improving the quality of care.
Qualifications
- Psychiatric experience preferred.
- Graduate of an accredited School of Nursing, Bachelor’s degree preferred.
- Current Connecticut RN License
- Excellent communication and critical thinking skills.
- Effective leadership and delegation skills.
- Experience with leading behavioral interventions or managing violent behaviors.

Job Description
Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager.
• Provides office-based nursing care in collaboration with provider
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention
• Demonstrates independent nursing actions based on assessment and problem identification
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
• Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy
Travel to other regional offices, as needed
Qualifications
Current state of Connecticut RN license and current CPR certification
Three years of experience in a medical practice preferred
Experience with medical software
