Contributions:

- Oversee/guide Program Specialists, Program Coordinators, and staff in areas of health management, health promotion, medication monitoring, administration, and documentation.

- Individual assessment, as needed, for acute and chronic medical concerns.

- Develop in-services, as needed, to educate Program Specialists, Program Coordinators, staff, and individuals when appropriate when new diagnosis/treatment plan is ordered to ensure that individual’s health care needs are met.
- Physical and phone assessment of individuals to address medical concerns reported by those working in the home; direct staff, Program Specialists, and Program Coordinators on course of action; and contact physician/pharmacy as needed.
- Attend medical appointments, as needed, to ensure that health care needs of the individuals are met.
- Communicate with individuals’ families and guardians, as needed, to address healthrelated concerns.
- Manage appropriate disease/medication protocols (lab/diagnostic testing) and ensure that recommended health maintenance procedures (colonoscopy, mammogram, Dexa Scan, etc.) are addressed.
- Review and complete Health Services review for all internal incident reports.
- Complete medical summary for all reportable incidents (including hospital and ER trips, follow-up appointments, etc.); email Incident Manager and obtain copies of hospital records for inclusion in individuals’ medical records/forward to Incident Manager for inclusion in incident file.
- Review all medical visitation forms to ensure that all recommendations and follow-up appointments/procedures are completed and that regulatory requirements are met.
- Maintain an appointment schedule for each individual to monitor that all regulatory and follow-up appointments are scheduled and attended. Coordinate physician visits.
- Maintain medical history for each individual including diagnoses, immunizations, diagnostic testing, and hospitalization/ER visits.
- Maintain current Medical Visit Record (updated as needed for changes in diagnosis, medication, allergies, insurance, etc.) and forward to Program Specialists so that most accurate information is available when individuals are taken to medical appointments.
- Maintain current Psych Medication Review form (as needed for individuals on Psychoactive medications).
- Complete Annual Physical form providing current/accurate diagnosis, medication, health maintenance, physical limitation, and diet information prior to physical. After physical, review form for completion by provider.
- Administration/interpretation of individuals’ Mantoux TB testing as needed.
- Review all medical/pharmacy bills for approval of payment and contact provider to dispute charges as needed.
- Communicate with Pharmacy (i.e., new orders, change in directions, ordering of additional doses) when needed. Return d/c medications as needed.
- Check all chronic medications with MAR every month (PRIOR to them being administered by staff). Compare current MAR to the following month’s MAR to ensure that all medications are continued.
- Review all MARs and Personal Care Records for the previous month to ensure that Medication Administration Class requirements for documentation have been followed, that all documentation on the MAR is completed, and forward list of needed corrections and recommended corrective action to Medication Practicum Observers, Program Director, Residential Manager, and Trainer.
- Maintain Medication Trainer status (through state-approved course); responsible for ongoing monitoring/certification of Medication Practicum Observers (Program Specialists/Coordinators); and assist with Weekly MAR reviews when Practicum Observers are on vacation, LOA, etc.
- Hepatitis B administration for staff.
- Perform other duties as required.

About the Units:

- 4 Main - Surgical oncology nursing encompasses patients who are recovering from post-surgical procedures for the treatment of life-altering cancers. These surgical procedures include cytoreductive (debulking) surgery and hyperthermic intraperitoneal chemotherapy treatment, whipple procedures, colon resections, liver resections, and other abdominal/rectal procedures. Nurses on 4main are exposed to many different types of tubes and drains that our patients experience in their post-surgical phase. These drains include (but are not limited to) wound vacs, ileostomies, colostomies, feeding tubes, jackson pratt (JP) drains, pigtail catheters, and chest tubes. Patients on 4main require staff members that are highly motivated, encouraging, and driven to provide patients with the best possible experiences and outcomes.
- 3 West - This is an Orthopedic unit that specializes in total joint replacement, orthopedic oncology surgical services, spine surgery, sports injuries, and a wide range of medical management. New and experienced nurses will experience an exciting range of services including wound care, surgical recovery, patient education, and the opportunity to see motivated patients work to get back on their feet through physical therapy and other nursing modalities. Our unit works collaboratively with all members of the care team and participates in “care coordination” where the nurse is a critical part of the patient’s discharge planning
- 3 Main - This is a 38-bed surgical unit caring for post-op cardiovascular and cardiothoracic patients. We care for high acuity patients recovering from open heart surgery, pacemaker insertion, aneurysms, and various thoracic and vascular surgeries. 3 Main a teaching unit with a dynamic learning environment, where nurses will gain experience with chest tubes, trachs, monitors, pacemakers, and arrhythmia IV drips. 3 Main is fast-paced and team-oriented, and serves as Shadyside's cardiac step-down unit.

Purpose:
Responsible for evaluating and treating children and adults with ocular motility disorders. Familiar with sensorimotor evaluation and measurement for pre and post surgical strabismus and other complaints relating to other binocular function. Patient care includes visual acuity testing, ocular motility testing, patient medical history and instruction to patients and guardians. The Certified Orthoptist must also instruct Ophthalmology residents, fellows and Ophthalmic Medical Technicians with regard to patient testing, data, testing techniques and theory. The Certified Orthoptist must determine testing methods most suitable for a particular patient. Data derived from these procedures are presented to the staffing Ophthalmologist/Optometrist to determine diagnosis and treatment plans for the patient. The Certified Orthoptist is expected to participate as a member of the Ophthalmology staff with various tasks as assigned.

- Will work in a multi-specialty practice that will refer patients to the Orthoptist for sensorimotor evaluation and treatment as a physician extender.

- Advising patients with low vision on the use of magnification and other strategies, such as lighting, to maximize their vision maintaining involvement in departmental research projects, the collection of clinical data for audits and the establishment of relevant protocols.

- Making a diagnosis and formulating a treatment plan, which might involve referral for surgery or the use of more conservative Orthoptic treatment, e.g. the use of a patch to treat a lazy eye; giving an explanation and full instructions to patients about the diagnosis and treatment program suggested.

- Undertaking general administrative duties relating to patient care as well as training students on placement and other health professionals, e.g. optometry students etc-

- Receiving patient referrals from the eye casualty and neurology departments, eye clinics, GPs, health visitors and community clinics; investigating patients clinically to assess their vision, eye position and eye movement, by observation and by the use of specialist equipment.

- The Orthoptist sees patients at the same site as the supervising ophthalmologist, or at a satellite office.
