The Behavioral Health Nurse provides assessments, support and care coordination across the behavioral health care continuum for patients struggling with mental illness as well as individuals and families seeking resources or information about treatment options. The Behavioral Health Nurse is an integral member of Behavioral Health Service Line and works collaboratively with the treatment team members and providers in the various inpatient and outpatient programs offered at Mercy. The Behavioral Health Nurse works closely with community partners to ensure successful transition of care and promotes education to the public on Behavioral Health-related topics through various forums. The Behavioral Health Nurse is responsible for carrying out duties as a health coach as well as utilizing skills as a psychiatric nurse in various care areas when needed, including but not limited to the Intensive Outpatient Program and Psych Triage in the Emergency Department.
What you will Do:

Join the MercyOne Family! We are looking for a Patient Placement Case Manager!
As a Patient Placement Case Manager at MercyOne, you will be responsible for the coordinating the placement of all admissions (elective and emergent) and transfers to MMC-NI in accordance with clinical standards and guidelines. The Patient Placement Case Manager will coordinate, prioritize, and optimize patient flow throughout the MMC-NI network, as well as coordinate, and facilitate patient entry into MMC-NI from other health care facilities/organizations. The Patient Placement Case Manager supports the Mission of Trinity Health and Mercy Medical Center-North Iowa.
Coordinates and directs patient flow to optimize access, capacity and support safe delivery of care.
Responsible for the coordination and facilitation of all patients requiring transfer from other healthcare facilities to MMC-NI by utilizing the Mercy First Call Centers process flows: (Communication/Teamwork)
Answers and responds to Mercy First Call line for patient admissions and transfer to MMC-NI by staying on the line and connecting the caller with the appropriate personnel as per protocol.
Facilitates ED to ED communication, Physician to Physician communication for transfers, admissions and consults.
Facilitates direct transfer of calls for trauma alerts, stroke alerts, and level I Cardiac patients to the appropriate personnel.
Facilitates communication between MMC-NI on call physicians and referring physicians to arrange acceptance of transferring patient.
Facilitates receiving admitting orders from admitting physicians if needed.
Communicates with the referring hospital to coordinate transportation arrangements and provide them with a bed assignment.
Accurately completes First Call Center Data base
Schedule:
The 0.6 FTE is noon to midnight with every third weekend and no holidays.
General Requirements:
Graduate of accredited school of nursing.
Current RN Licensure in the state of Iowa.

MercyOne North Iowa is the largest provider of health care services in our region, and we are proudly the largest employer in Cerro Gordo County. Our hospital is a private, not-for-profit health care center, licensed for 346 beds, which serves more than 400,000 inpatients and outpatients each year. In addition, MercyOne North Iowa is a teaching institution. We conduct a Family Medicine Residency, Internal Medicine Residency, Cardiology Fellowship, Hospice and Palliative Medicine Fellowship, PGY1 Pharmacy Residency and a School of Radiologic Technology. As a Clinic Registered Nurse at MercyOne, you will provide patient care in outpatient settings and are responsible for delivering safe, effective, and evidence-based patient care through assessment, planning, implementation, and evaluation. They coordinate care provided by other clinical staff, support patient and family education, and ensure communication across healthcare teams. They are accountable for clinical standards and may operate with minimal supervision.
Schedule:
- Full time 40 hours a week
- 8:00a-5:00p
- No Weekends No Holidays
General Requirements:
- Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
- Plans, assesses, intervenes, and provides direct patient care using critical thinking and nursing processes (assessment, planning, implementation, evaluation).
- Performs comprehensive patient assessments (physical, psychosocial, nutritional, functional, safety, spiritual, cultural, educational). Makes nursing decisions based on patient status and care needs.
- Administers medications and treatments safely via multiple routes (oral, IM, SC, ID, IV, inhalation, topical, ocular, optic, sublingual, nasal) and documents accordingly.
- Performs nursing care, lab and diagnostic tests, specimen collection, phlebotomy, and limited radiology procedures following policies and universal precautions.
- Assists providers with patient care, phone/mail/fax communications, scheduling, consents, admissions, referrals, and outpatient procedures.
- Communicates and collaborates with physicians, staff, patients, families, and interdisciplinary team members regarding care plans and patient needs.
- Educates patients, families, and clinical staff on treatments, medications, test preparation, discharge, and health maintenance; documents education and evaluates understanding.
- Participates in staff meetings, committees, quality improvement, risk management, and performance improvement projects.
- Maintains documentation accuracy and completeness in EMR: assessments, vitals, symptoms, treatments, medications, education, charges, lab results, histories, and consults.
- Maintains and monitors supplies, equipment, medication stock, and office machines; follows maintenance and quality control protocols; reports malfunctions.
- Recognizes and responds appropriately to urgent/emergency patient needs and initiates emergency procedures.
- Other duties as needed and assigned by the manager.
- Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
Education:
- Must be comfortable operating in a collaborative, shared leadership environment.
- Active RN License in the state of Iowa; Licensure from other states may be required based on location. Compact State License acceptable where applicable.
- Basic Life Support (BLS) certification required within 3 months of hire.
- Proof of completion of Mandatory Reporter abuse training specific to the population served within 3 months of hire.
- Proficient in basic computer skills (Windows environment, web-based navigation, email, MS Office).
- Ability to work with diverse groups of people in a collaborative, shared leadership environment.
- Requires the ability to read, write and understand the English language, and communicate effectively with patient, visitors, and colleagues while performing their job duties.
- Completion of specialty classes or proof of other certifications required for some roles.
