JOB SUMMARY:

RESPONSIBILITIES:
- Perform assessments of residents' physical, emotional, and medical needs; monitor health status and report changes as needed.
- Administer medications and treatments (oral, IV, topical, injectable, as permitted) and perform skilled nursing procedures per physician orders and facility policy.
- Monitor and record residents' vital signs, perform regular health checks, observe for changes or signs of concern, and respond appropriately to changes in condition or emergencies.
- Provide direct nursing care and support residents with activities of daily living (as needed), hygiene, mobility assistance, and other care needs.
- Collaborate with the care team (therapists, CNAs/LPNs, social work, physicians) to ensure cohesive, resident-centered care.
- Maintain accurate, timely documentation of assessments, treatments, care provided, changes in condition, and communications in residents' medical records.
- Communicate with residents, families/POAs, and the care team regarding resident status, care plans, treatments, and any changes or concerns.
- Provide education and support to residents and their families regarding care, treatments, medications, and wellness (as appropriate).
QUALIFICATIONS:
- Valid, unrestricted RN license in Connecticut.
- Graduate of an accredited nursing program (ADN or BSN) - experience in long-term care or skilled nursing preferred.
- Current CPR/BLS (or equivalent) certification as required by facility policy.
- Strong clinical assessment skills, compassion, and a resident-centered approach; excellent communication and interpersonal skills.
- Ability to work in a long-term care or skilled nursing environment; reliable attendance and ability to work collaboratively as part of a care team.
- Capacity to perform nursing procedures (medication administration, IV therapy, wound care), monitor residents' conditions, and respond appropriately to changes or emergencies.

- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and personal hygiene.

- Take and record residents' vital signs (e.g. temperature, pulse, blood pressure, respiration) and monitor for changes in condition.
- Serve meals and assist with feeding residents who need help; monitor and record intake and output as required.
- Provide basic room care: change bed linens, clean and maintain resident rooms and common areas to ensure a safe, sanitary environment.
- Assist with incontinence care, toileting, and maintaining resident dignity and comfort.
- Observe residents' physical and emotional status; provide companionship and emotional support; alert supervising nurse to any changes or concerns.
- Communicate with residents, families/POAs, and the care team about resident needs, status, and any observed changes.
- Follow all facility policies including infection control, safety procedures, and resident confidentiality.
QUALIFICATIONS:
- Valid CNA certification required for Connecticut.
- Ability to read, write, speak, and understand English.
- Compassionate, resident-centered attitude; excellent interpersonal and communication skills.
- Physical stamina and ability to lift, turn, move, and assist residents - including bending, standing, and walking as required by resident care.

QUALIFICATIONS:
- Valid, unrestricted LPN license in Connecticut.
- Graduate of an accredited practical/vocational nursing program.
- Current CPR/BLS (or equivalent) certification as required by facility policy.

- Take and record residents' vital signs (blood pressure, temperature, pulse, respiration) and monitor for changes in condition.
- Assist residents with activities of daily living (ADLs) as needed, including bathing, dressing, feeding, mobility, and hygiene.
- Perform basic wound care, dressing changes, catheter or ostomy care (as allowed by facility policy).
- Collect specimens for lab tests (e.g. urine, sputum) as ordered.
- Observe and document residents' physical and emotional status and report any changes to the supervising RN or physician.
- Maintain accurate and timely documentation of care provided, treatments administered, and resident observations in medical records.
- Communicate with residents, families/POAs, and the healthcare team about resident status, care needs, and any concerns.
- Adhere to infection control protocols, facility policies, and standards of care.
QUALIFICATIONS:
- Valid, unrestricted LPN license in Connecticut.
- Graduate of an accredited practical/vocational nursing program.
- Current CPR/BLS (or equivalent) certification as required by facility policy.
- Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.
- Compassionate, resident-centered approach, with good organizational and time-management skills.
- Comfortable working in a long-term care or skilled nursing environment (experience preferred).

Job Summary
Clinical Supervisor of Behavioral Health oversees a team of 5 full time master's level clinicians, a case manager, and a recovery support specialist who provide treatment within an acute inpatient psychiatric unit.Β Partnering in multidisciplinary leadership with a nurse manager and medical director to provide programmatic oversight of the unit and treatment for patients 18 years of age and older.Β Additional responsibilities include providing clinical supervision to the clinical team, supporting the clinical development of clinical trainees placed within the inpatient units, and engaging in quality improvement projects at the direction of the clinical director in accordance with best practices and regulatory requirements.Β Clinical services include individual, group and family therapy.Β

Qualifications
Master's degree in social work, counseling or marriage and family therapy required.
Current Connecticut license; LCSW, LPC, LMFT required.
1 year of management experience in a healthcare related setting.

2 years of experience as a licensed Clinician; LCSW, LPC, LMFT.
Previous experience in clinical setting with Adults or Child/Adolescents
Computer proficient
Ability to assess problems, determine solutions and make decisions within a timely manner
Utilize supervision and team approaches in the context of patient assessment and clinical interventions
Excellent communication skills, with the ability to establish relationships within and outside their entity
Focused on personal, patient/client and unit/staff safety

Program Summary
Natchaug Hospital provides a wide range of psychiatric and substance-use treatment for children, adolescents and adults at our 57-bed Mansfield Center inpatient facility. The highly qualified, experienced and dedicated professional staff of Natchaug Hospital provides expert care in a supportive, therapeutic setting β helping people regain their stability and return as soon as possible to their own homes.
Job Summary
Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in theΒ InpatientΒ department. Clinical services include individual, group and family therapy.Β
Key responsibilities of Behavioral Health Clinician:Β
- Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.Β
- Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.Β
- Refers patient to appropriate levels of care and available resources that promote the individualized care plan.Β
- Maintains timely patient-centered documentation thatΒ reflect the patient and client systemsβ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.Β Β

Qualifications
- Masterβs degree in social work, counseling, or marriage and family therapy or related human service field.
- Previous experience in a healthcare setting required, preferably working with a mental health and/or chemical dependency population
- Current Connecticut LMSW, LPC-A, LMFT-A, or CADCΒ
- Excellent communication skills, with the ability to establish relationships within and outside their entity
