CARE COORDINATION: The RN Case Manager works with the multidisciplinary team including physicians, staff, and payers to ensure patient's progress along the continuum of care in an efficient and cost-effective manner that ensures quality outcomes. The RN Case Manager acts as a patient advocate and communication link with other departments and community resources. Supports nursing staff in development of individualized, evidence-based, outcomes oriented, and safe practice
Responsible for patient assessment of complex clinical discharge needs and establishment of an appropriate plan of care with multidisciplinary team. Demonstrates critical thinking skills and oversees the care coordination of defined population. Ensures that patient goals are set with patient/family, negotiates healthcare resources and educates patient/family regarding available options of care throughout the continuum. Demonstrates clinical competence to care for a specific patient population. Facilitate patient education for safe and effective discharge.
PROFESSIONAL DEVELOPMENT: Demonstrates leadership behaviors that enhance and support team functions. Mentors and coaches staff. Maintains competencies and participates in continuing education for self and development of colleagues and students. Participates in delivering professional programs/ courses/in-services, internal and/or external, and community events. What You’ll Do
- Support diagnostic and interventional cardiac and peripheral vascular procedures following sterile technique
- Operate fluoroscopy, C‑arm, PACS, and hemodynamic monitoring systems
- Prepare procedure rooms, position patients, and ensure safe and accurate imaging
- Assist physicians during Cath Lab cases, monitor vital signs, and contribute to optimal procedural flow
- Participate in the on‑call rotation, supporting emergent cases when needed

- Flexibility required for on‑call and occasional extended shifts depending on case volume
