EDUCATION/EXPERIENCE
Two years experience as a physical therapist is required. Supervisory and/or clinical teaching experience and additional clinical experience is preferred. Meets clinical and leadership activities and behaviors as defined in the clinical ladder or equivalent.
LICENSURE
Licensure by the Executive Council of Physical and Occupational Therapy Examiners as an Physical Therapist is required. Must have a current AHA BLS Healthcare Provider or AHA BLS Instructor Provider card. Ambulatory Therapist must be a participating Medicaid and Medicare provider or be eligible to obtain a Medicaid and Medicare provider number.
