Position Summary: Performs tasks to maintain cleanliness and appearance by following departmental procedures and using equipment properly. Work involves stripping, re‐waxing and upkeep of all hard surface floors; shampooing all carpeted surfaces on a routine basis; occasionally moving furniture. Areas included are exam rooms, offices, bathrooms, corridors, reception area and lobby. Education, License & Cert: High school graduate preferred. Experience: Previous experience in housekeeping helpful. Must be able to follow oral and written instructions. On the job training is provided. Essential Functions: 1. Responsible for shampooing all carpeted surfaces on a regular, periodic basis. 2. Performs tasks to maintain the building in a sanitary, orderly, and attractive condition. 3. After receiving instructions as to specific work assignment, assembles necessary cleaning supplies and equipment for transporting to the designated area. Areas included, but not limited to: Exam rooms, offices, bathrooms, corridors, reception areas and lobby. 4. Determines the proper dilution ratio of chemicals and administers the correct amount of chemical needed to complete assigned tasks. 5. On occasion this job may be combined with moving furniture, wall washing, window washing, and spot cleaning, as necessary. 6. Reports any maintenance, security, or other problems to the supervisor. Turns off all lights when not used. Keeps doors locked at all times. 7. Demonstrates proficiency in operating and maintaining equipment including high‐speed burnisher, buffer, extractor, wet vac, and vacuum. 8. Assesses individual floor maintenance needs on a daily basis to plan and prioritize work. 9. Maintains log of activity for reference and planning. Other Duties: 1. Performs other duties as assigned, including but not limited to moves for regional offices, moving and/or assembling furniture.

Position Summary:
The incumbent shall be responsible for providing quality care to medical and surgical cardiovascular patients of cardiac rehab services by supporting the development and implementation of a comprehensive Cardiac Rehabilitation Phase I ‐ III program that involves the patient and family. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary manner.
Education, License & Cert:
Prefer graduate from an accredited four ‐ year Bachelor’s degree program in Exercise Physiology / Physical Education or related field. However, may consider coursework in exercise physiology with direct patient care experience in lieu of Bachelor’s degree.
Experience:
Education/experience in developing safe exercise prescription and education for the cardiac patients is preferred.
Essential Functions:
1. Communicates and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others.
2. If applicable, conduct and document in EPIC the initial Phase I review to include medical record review and patient interview to effectively communicate education for risk factor modification based on individual risk factor history. Determine appropriateness of Phase II referral.
3. Provide appropriate Phase I‐III exercise prescription resulting in positive outcomes as indicated by quality monitoring and patient satisfaction.
4. Evaluate and document patient response to treatment. Patient response to the treatment plan is evaluated by patient achievement of goals with update of goals as needed. Collaborate with Cardiac Rehab nurse. Provide information to patient/family regarding status, program planning and discharge plans throughout the rehabilitation program.
5. Document the course of patient care including progress made to justify need for ongoing intervention. Follow documentation guidelines for the inpatient and outpatient Cardiac Rehab environment according to the department policy/procedure/guidelines.
6. Provide patient and family education on risk factor modification and exercise guidelines. Integrates family members into the treatment, educational and discharge planning process for a safe transition back to home. Educational methods are adapted to meet patient and family education needs.
7. Participating in continuing education/professional development activities. Attends/presents at departmental in‐service sessions as scheduled. Attends external professional growth activities as scheduled and communicates information to the team.
8. Adhere to policies and practices of applicable federal and state guideline, and entity specific policies. Is familiar with and acts according to contents of applicable professional code, employee/handbook, unit Policy & Procedure and organizational policies.
9. Meets established organizational productivity standards within a fiscally responsible manner.
10. Participates in performance improvement and quality related initiatives and uses data to improve quality of care delivered.

  • No more than two (2) C-sections
  • Not receiving government or public assistance (including Medicaid, WIC, SNAP/Food Stamps, or state-funded health insurance)
  • No history of pregnancy complications (including gestational diabetes, pre-eclampsia, or pre-term labor)
  • No history of schizophrenia, bipolar disorder, borderline personality disorder, or major depressive disorder

Compensation & Benefits

We recognize the commitment, time, and care involved in being a gestational carrier. Compensation includes:

Base Compensation

  • $60,000 – $70,000
