You?ll begin with a 90-day onboarding period, including training and gradual caseload build, supported by 2 full-time Health Coaches and a dedicated admin team.
Responsibilities
- Deliver 1:1 patient care (45-minute sessions, in person and virtual)
- Conduct detailed functional assessments, interpret labs, and design personalized plans
- Collaborate with coaches to implement clinical recommendations and track progress
- Maintain accurate documentation and communication in our EMR

Ideal Candidate
- Licensed Nurse Practitioner (NP), Physician Assistant (PA), or Physical Therapist (PT) or similar
